Lymphangioma is a benign disfiguring congenital lymphatic system malformation. It mainly occurs in paediatric age group and mostly present in the head and neck region. It can pose a very challenging situation as it can infiltrate many adjacent vital structures. Occasionally, it also presents with complications.We report a case of a three-year old girl presented initially with an infected giant multicystic cervical lymphangioma which subsequently ruptured spontaneously. We attempted initial compression bandage, however converted into complete surgical excision as the leak continued. It was a successful operation without recurrence.
