Patients with Attention Deficit Hyperactivity Disorder (ADHD) suffer not only from inability to focus but also from inability to shift attention for events that trigger their interests. This phenomenon is called "hyperfocusing". Previous literature about hyperfocusing is scarce and relies mainly on case reports. The study aimed to investigate and compare the severity of hyperfocusing in adult ADHD with and without psycho-stimulant use. ADHD (DSM-IV-TR) patients either psycho-stimulant naive (n=53) or on psycho-stimulants (n=79) from two ADHD clinics were recruited. The control group (n=65) consisted of healthy university students. A socio-demographic form, the Beck Depression Inventory, the Wender-Utah Rating Scale, the Adult ADHD Self- Report Scale and the Hyperfocusing Scale were applied to the participants. There was no difference between total Hyperfocusing Scale and Adult ADHD Self- Report Scale scores of two patient groups, but both have higher scores than controls (p<0.001). Hyperfocusing is higher in adult ADHD and there was no difference between stimulant-naive patients or patients on stimulants. Hyperfocusing can be defined as a separate dimension of adult ADHD.

Airway Management of a Patient with an Acute Floor of the Mouth Hematoma after Dental Implant Surgery in the Lower Jaw

BackgroundOver the last decades, dental implants have become increasingly popular in the prosthetic rehabilitation of patients. This has subsequently led to an increase of perioperative complications. Obstruction of the airway as a result of a floor of mouth hematoma after dental implant surgery is a rare but life-threatening complication.Case ReportA 62-year-old man presented to the emergency department with a compromised airway caused by a hematoma in the floor of the mouth that occurred during dental implant surgery in the edentulous anterior mandible. Computed tomography angiography images revealed an elevation of the floor of mouth with subsequent occlusion of the airway. In addition, a perforation of the lingual mandibular cortical plate was observed that was caused by two malpositioned dental implants. Awake fiberoptic intubation was immediately performed, the two malpositioned dental implants were subsequently removed, and the patient was extubated after 3 days.Why Should an Emergency Physician Be Aware of This?Perforation of the lingual mandibular cortical plate during dental implant surgery can lead to life-threatening bleeding in the floor of the mouth. This condition can be successfully treated by awake fiberoptic intubation and, if necessary, the malpositioned dental implants can be subsequently removed.

Effect of a Multi-Diagnosis Observation Unit on Emergency Department Length of Stay and Inpatient Admission Rate at Two Canadian Hospitals

BackgroundObservation units (OUs) have been shown to reduce emergency department (ED) lengths of stay (LOS) and admissions. Most published studies have been on OUs managing single complaints.ObjectiveOur aim was to determine whether an OU reduces ED LOS and hospital admission rates for adults with a variety of presenting complaints.MethodsWe comparatively evaluated two hospitals in British Columbia, Canada (hereafter ED A and ED B) using a pre–post design. Data were extracted from administrative databases. The post-OU cohort included all adults presenting 6 months after OU implementation. The pre-OU cohort included all adults presenting in the same 6-month period 1 year before OU implementation.ResultsThere were 109,625 patient visits during the study period. Of the 56,832 visits during the post-OU period (27,512 to ED A and 29,318 to ED B), 1.9% were managed in the OU in ED A and 1.4% in ED B. Implementation was associated with an increase in the median ED LOS at ED A (179.0 min pre vs. 192.0 min post [+13.0 min]; p < 0.001; mean difference −12.5 min, 95% confidence interval [CI] −15.2 to −9.9 min), but no change at ED B (182.0 min pre vs. 182.0 min post; p = 0.55; mean difference +2.0 min, 95% CI −0.7 to +4.7 min). Implementation significantly decreased the hospital admission rate for ED A (17.8% pre to 17.0% post [−0.8%], 95% CI −0.18% to 0.15%; p < 0.05) and did not significantly change the hospital admission rate at ED B (18.9% pre to 18.3% post [−0.6%], 95% CI −1.19% to −0.09%; p = 0.09).ConclusionsA multi-diagnosis OU can reduce hospital admission rate in a site-specific manner. In contrast to previous studies, we did not find that an OU reduced ED LOS. Further research is needed to determine whether OUs can reduce ED overcrowding.

Catamenial Epilepsy: The Menstrual Cycle as a Clue to Predict Future Refractory Seizures

It is estimated that more than one-third of women with epilepsy experience catamenial seizure exacerbation.Case ReportWe present the case of a young female patient who presented to the emergency department with an increase in seizure frequency that coincided with her menstrual cycle, despite complete medication compliance.Why Should an Emergency Physician Be Aware of This?It is important for the emergency physician to consider catamenial epilepsy in the differential diagnosis for secondary causes of seizure to ensure appropriate follow-up, as well as improve the quality of life of patients suffering from uncontrolled seizures.

Radial Head Subluxation: Factors Associated with Its Recurrence and Radiographic Evaluation in a Tertiary Pediatric Emergency Department

BackgroundRadial head subluxation (RHS) is a common complaint seen in the pediatric emergency department in children ages 6 months to 4 years. Classically, injury occurs due to axial traction on the arm, but this mechanism is not universal. Some patients will have recurrent RHS; some may undergo x-ray (XR) evaluation for alternative diagnosis.ObjectivesTo determine factors associated with recurrences and radiographic evaluations in RHS.MethodsA retrospective study with inclusion criteria: under 10 years of age with discharge diagnosis "nursemaid," "radial head," or "subluxation." We examined factors associated with RHS recurrences, circumstances when radiographic evaluations performed, physician's training background (pediatric vs. general emergency medicine), mechanisms of injury, and demographic factors including age, gender, and arm involved.ResultsIn 246 visits, median age was 27 months (interquartile range 16.1), with females comprising 55.7% (n = 137), and left-sided predominance (52%, n = 130). Mechanisms of injury were classified as "pull" (65.9%, n = 162), "non-pull" (15.9%, n = 39), and "unknown" (18.3%, n = 45). Eighteen patients with recurring RHS were more likely to be male (p = 0.008). In 61 visits where radiography was performed, patients were older (p = 0.03), with a higher frequency seen in non-pull and unknown mechanism (p = 0.0001). No significant difference was found in frequency of radiographs obtained in regard to physician training (p = 0.4660).ConclusionRHS can result from a myriad of mechanisms. We found that recurrence was more likely in male patients. Factors associated with radiographic evaluation included atypical mechanism, older age, and unclear history, regardless of physician training background.

Pituitary function within the first year after traumatic brain injury or subarachnoid haemorrhage
Abstract
Purpose
Reports on long-term variations in pituitary function after traumatic brain injury (TBI) and subarachnoid haemorrhage (SAH) diverge. The aim of the current study was to evaluate the prevalence and changes in pituitary function during the first year after moderate and severe TBI and SAH and to explore the relation between pituitary function and injury variables.
Methods
Adults with moderate and severe TBI or SAH were evaluated at 10 days, 3, 6 and 12 months post-injury/illness. Demographic, clinical, radiological, laboratory, including hormonal data were collected.
Results
A total of 91 adults, 56 (15 women/41 men) with TBI and 35 (27 women/8 men) with SAH were included. Perturbations in pituitary function were frequent early after the event but declined during the first year of follow-up. The most frequent deficiency was hypogonadotrope hypogonadism which was seen in approximately 25 % of the patients. Most of the variations were transient and without clinical significance. At 12 months, two patients were on replacement with hydrocortisone, four men on testosterone and one man on replacement with growth hormone. No relations were seen between hormonal levels and injury variables.
Conclusions
Perturbations in pituitary function continue to occur during the first year after TBI and SAH, but only a few patients need replacement therapy. Our study could not identify a marker of increased risk of pituitary dysfunction that could guide routine screening. However, data demonstrate the need for systematic follow-up of pituitary function after moderate or severe TBI or SAH.

Cross-cultural and socio-demographic correlates of homophobic attitude among university students in three European countries
Abstract
Purpose
The aim of this study was to investigate homophobic attitudes in three European countries: Italy, Albania, and Ukraine. One thousand and forty-eight students were recruited in Italian (n = 766), Albanian (n = 180), and Ukrainian (n = 102) university centers.
Methods
A socio-demographic questionnaire and Homophobia Scale (HS) were administered by our staff.
Results
Cross-cultural and significant differences among Italian, Albanian, and Ukrainian students were found on the Homophobia Scale (HS; Italy: mean = 22.26 ± 16.73; Albania: mean = 38.15 ± 17.28; Ukraine: mean = 59.18 ± 16.23). The analysis of socio-demographic characteristics revealed that the male gender emerged as main predictor of homophobic attitude in all the three countries, although also a conservative political orientation and the religious belief predict higher homophobia levels in Italy and Albania, particularly.
Conclusions
This study revealed that in these European countries assessed, attitudes toward homosexuality are different. Ukrainians display higher levels of homophobia than Albanians and Italians, confirming the central role of cultural differences in homophobic attitudes. Nevertheless, some socio-demographic aspects such as identification as male have a similar influence on homophobic attitudes in all assessed populations.

Freestyle local perforator flaps for facial reconstruction: clinical experience and complications
Abstract
Background
Reconstruction of facial defects always represents a surgical challenge as functional and cosmetic outcomes must be taken into account more than any region of the body. The concept of freestyle perforator flaps has been developed to obtain a complete range of freedom in their movement to reach the defect. We present our clinical experience with facial perforator flaps, focusing on indications, surgical technique, and complications.
Methods
Thirty-one facial defects were reconstructed with freestyle local perforator flaps between January 2007 and November 2014. Doppler identification of perforator vessels preceded preoperative planning and the flap harvesting followed the dissection of perforator vessels.
Results
Twenty-two clinical cases had no complications. Four had venous congestion that resolved spontaneously, three had a distal 1/3 superficial necrosis, one suffered hematoma, and one had a 1/5 distal area of full thickness necrosis.
Conclusions
Freestyle perforator approach was applied to gain more freedom for facial reconstruction, allowing one-stage procedures and low donor site morbidity. Good anatomical understanding, precise planning, and meticulous technique can affect clinical results of perforator flaps in the face. A critical approach is essential to get better results.

Microsurgical flaps in the treatment of thoracic radionecrosis: a case series
Abstract
Background
Radionecrosis of the thoracic wall is a late complication of radiotherapy mainly found in breast cancer patients. During the treatment of severe thoracic radionecrosis, the aggressive ablation of a large amount of damaged tissue is fundamental, making the use of healthy remote tissue provided by a free flap essential. However, most of the papers focusing in the reconstruction of this entity are dated or case reports. This article is intended to present the utility and results of reconstructive microsurgical techniques in these patients.
Methods
We carried out a retrospective review of the patients who underwent microsurgical reconstruction of extensive thoracic defects associated with radionecrosis in the period 2007 to 2012 in our centre. The variables studied were sex, age, size of the lesion, type of reconstructive procedure used, the postoperative evolution of the flap and the donor area, and the length of the hospital stay.
Results
Four patients with extensive thoracic defects associated with radionecrosis were included. After debridement, bone reconstruction was performed using a polytetrafluorethylene mesh, while soft tissue reconstruction was achieved using a free flap. One of the patients was reconstructed with a deep inferior epigastric perforator free (DIEP) flap and the rest with transverse myocutaneous gracilis (TMG) free flap. No flap-related complications were observed during the postoperative period.
Conclusions
The effects of radiation on soft tissues and bones are the cause of the increasing complexity of thoracic reconstruction. We believe that the use of free flaps to provide large amounts of tissue or cover particularly problematic areas can be an extremely useful tool for these patients.

Analysis of Practice Settings for Craniofacial Surgery Fellowship Graduates in North America

ObjectiveIn North America, the number of craniofacial surgery fellowship graduates is increasing, yet an analysis of practice settings upon graduation is lacking. We characterize the practice types of recent graduates of craniofacial fellowship programs in the United States and Canada.DesignA 6-year cohort of craniofacial fellows in the United States and Canada (2010-2016) were obtained from craniofacial programs recognized by the American Society of Craniofacial Surgery. Practice setting was determined at 1 and 3 years of postgraduation, and predictors of practice setting were determined.ResultsA total of 175 craniofacial surgeons were trained at 35 fellowship programs. At 1 year of postgraduation, 33.6% had an academic craniofacial position and 27.1% were in private practice (p = 0.361). A minority of graduates pursued additional fellowships (16.4%), nonacademic craniofacial positions (10.0%), academic noncraniofacial positions (5.7%), and international practices (7.1%). At 3 years of postgraduation, the percentage of graduates in academic craniofacial positions was unchanged (34.5% vs 33.6%, p = 0.790). The strongest predictors of future academic craniofacial practice were completing plastic surgery residency at a program with a craniofacial fellowship program (odds ratio = 6.78, p < 0.001) and completing an academic craniofacial fellowship program (odds ratio = 4.48, p = 0.020).ConclusionsA minority of craniofacial fellowship graduates practice academic craniofacial surgery. A strong academic craniofacial surgery background during residency and fellowship is associated with a future career in academic craniofacial surgery. These data may assist trainees choose training programs that align with career goals and educators select future academic surgeons.

ObjectivesTo develop a novel assessment tool for trainees-led trauma resuscitation. Assess psychometric properties of the proposed tool. Evaluate feasibility and utility of the tool.IntroductionTrauma resuscitation is a structured and complex process involving unique sets of skills. There is currently no published structured formative evaluation tool for trauma trainees. Therefore, many trauma trainees rely upon limited, unstructured feedback on their performance. We developed a tool to assess trainee performance while leading a trauma resuscitation and to assist faculty in providing trainee feedback after the encounter.MethodsThis study was conducted in a level I trauma centre in Ontario, Canada. Principles of learning theories, literature review, and clinical expert opinions were used to design a tool to assess clinical competence required to lead the resuscitation. In total, 5 critical domains were identified. High-fidelity simulation-based environment was used to test interrater reliability using intraclass correlation coefficients. To gauge feasibility, practicality, and utility of the tool, an online survey was sent to raters and trainees at the end of the study.ResultsWe found "excellent" agreement for "initial critical assessment" domain (0.80) and "moderate to good" agreement for the "communication and leadership" (0.67) and "clinical performance" domains (0.53). "Poor" agreement was identified for the "decision-making" domain (0.33). The coefficients for individual items reached "good" agreement for 5 items, and "moderate" agreement for 8 items. Intraclass correlation coefficients for the remaining 7 items were "fair" or "poor." Most raters agreed that items in the medical training domain were not applicable. Feedback from raters and trainees confirmed the feasibility and acceptability of the tool for formative feedback, in addition to some suggestions to enhance the tool.ConclusionMacTrauma TTL assessment tool is a novel tool for formative feedback for trainees' performance during trauma resuscitation. Initial psychometric property testing is promising. Further reliability and validity testing of the modified tool is needed. The tool has been shown to be feasible and acceptable by both trainees and faculty as a formative assessment tool.

Correlation of microvessel parameters in invasive ductal carcinoma breast and fibroadenomas: a morphometric study

IntroductionModifications of microvascular configuration are essential features encountered during the progression of breast tumours. Our objectives were to correlate morphometrically evaluated microvessel parameters [microvessel density(MVD), microvessel caliber(VC), microvessel cross sectional area(VCSA), percentage of total VCSA (%TVCSA) and total microvessel boundary density(TVBD)] with histological grades of invasive ductal carcinoma breast(IDC) and benign breast lesions.Materials and MethodsSixty cases of IDC presented with modified radical mastectomy and 20 benign breast fibro-adenomas were evaluated for various microvessel parameters, using CD34-immunostained histological sections by computerized image morphometry. Samples were divided into four histological groups: Benign, Grade1, Grade2 and Grade3; mean with standard deviation and range was evaluated for each group.ResultsHistological grades showed strong positive correlation with %TVCSA(ρ=0.773) and TVBD(ρ=0.811), moderate positive correlation with MVD(ρ=0.607), VC(ρ=0.609) and VCSA(ρ=0.616) when analyzed for all samples of the four groups. Except MVD, all parameters including age was the lowest (P<.001) for the benign group. Among the IDCs, differences of mean VC and VCSA were not significant; MVD, %TVCSA and TVBD were the lowest in Grade1 and the highest in Grade3. Upper cutoff values of benign lesions for: MVD was 155mm−2, VC was 9.94μm, VCSA was 94.42μm2, %TVCSA was 1.33 and TVBD was 4.37mm−1.ConclusionTVBD included the information of microvessels concentration and size showed the best correlation with grades. MVD showed positive correlation with grades in the IDCs; but for the differentiation of benign from malignant, VC, VCSA, %TVCSA and TVBD showed excellent area under the ROC curve (AUC>0.990), unlike MVD (AUC=0.797).

GATA3 expression in clinically useful groups of breast carcinoma: a comparison with GCDFP15 and mammaglobin for identifying paired primary and metastatic tumors

GATA3 has been recognized as the novel marker for identifying primary and metastatic breast carcinomas, consistently showing that GATA3 was significantly more sensitive than traditional marker GCDFP15 and Mammaglobin (MGB). However, clinically useful groups of breast carcinomas status were not identified, which was determining appropriate treatment strategy, affecting the prognosis. In this study, we undertook a comparative study of the marker GATA3 and GCDFP15 and MGB in clinically useful groups of paired primary and metastatic breast cancer. We retrieved 64 cases of matched primary and metastatic breast cancer from the surgical pathology archive at our institution. According to the emerging St. Gallen 2015 consensus, the clinically useful groups were divided into ER and/or PR (+), HER2 (−), abbreviated as A; ER and/or PR (+), HER2 (+), abbreviated as B; ER and PR (−), HER2 (+), abbreviated as C; ER, PR and HER2 (−), abbreviated as D; each group contained 16 cases (n=16). Tissue microarrays were created, with three 1-mm punch specimens from each case. The tissue microarrays were cut at a 4-μm thickness and stained with monoclonal antibodies to GATA3, GCDFP15 and MGB. Staining intensity (0~3+) and extent (0% to 100%) were scored with an H-score calculated (range, 0 to 300). Sensitivities by varying H-score cutoffs (any; ≥50; ≥150) for a positive result in the clinically useful groups of matched primary or metastatic breast cancer among GATA3, GCDFP15 and MGB. GATA3 was significantly more sensitive than GCDFP15 and MGB in a majority of paired primary or metastatic breast cancer groups, including all groups, A and B groups (P<.05) rather than C and D groups (P>.05). However, GATA3 in conjunction with GCDFP15 and MGB detection could improve the sensitivity of C group (P<.05) rather than D group (P>.05). Significantly, good coincidence was observed between primary and metastatic tumor GATA3 expression (kappa value =0.826>0.75) as compared with the coincidence of GCDFP15 (kappa value =0.492<0.75) and MGB (kappa value =0.593<0.75) (both P<.05). In conclusion, GATA3 expression did not show the same sensitivity for the clinically useful groups of breast cancer. GATA3 expression is positively correlated with ER-positive, PR-positive, and HER2-positive carcinomas. In addition, the matched primary and metastatic tumor expression of GATA3 show good coincidence. We propose the careful selection of GATA3 for identifying hormone receptor-negativity of breast cancer, especially in the case of triple-negative breast cancer.

Dermatofibrosarcoma protuberans (DFSP), the most common dermal sarcoma, is a malignant fibroblastic tumor most frequently arising in middle-aged adults. It is typically a low-grade sarcoma that grows slowly but has a high rate of local recurrence with low metastatic potential. DFSP is characterized by a specific translocation t(17;22)(q22;q13) leading to formation of COL1A1-PDGFB fusion transcripts. Histologically DFSP has characteristic morphology, of storiform islands of bland spindle cells, and immunohistochemically it shows diffuse expression of CD34. However, the morphology and immunoprofile can overlap with a variety of other soft tissue neoplasms. The preferred management of localized disease is wide surgical resection or Mohs micrographic surgery, while radiotherapy may be used for margin-positive disease where re-excision is not possible, or for inoperable disease. DFSP is generally regarded as refractory to conventional chemotherapy. Treatment options for systemic disease have been previously limited, but the PDGFβR, KIT and ABL inhibitor imatinib is now an option for effective systemic therapy. Continued insight into the tumorigenic molecular changes generated by the fusion oncogene may lead to further specific targeted treatments. We review DFSP, discussing the morphologic spectrum and variants, immunohistochemistry, molecular genetic findings, potential targeted treatments and the differential diagnosis.

A Comparison of Cancer Stem Cell Markers and Non-classical MHC Antigens in Colorectal Tumor and Non-Cancerous Tissues

Colorectal carcinoma (CRC) is one of the most fatal types of cancer in both women and men, and, unfortunately, patients are often diagnosed at an advanced stage. Cancer stem cells (CSCs) are associated with poor prognosis, metastasis and recurrence, as well as chemotherapy and radiotherapy resistance. Therefore, different treatment alternatives are needed to facilitate the elimination of CSCs. One such approach is immunotherapy; however, tumor cells can evade immune cells by alteration of the expression patterns of human leukocyte antigens (HLA). In this study, we immunohistochemically investigated the expression patterns of CSC-specific markers CD44, CD133, Nanog, and Oct3/4, and immunosuppressive molecules HLA-G and E in advanced CRC tumor tissues and non-cancerous colon biopsies. We found significantly increased CD44, Nanog, Oct3/4, HLA-G, and HLA-E expression in the CRC tumor tissues, compared to the non-cancerous colon biopsies. These findings suggest that some tumor cells may be CSC-like, and that the increased expression of HLA-G and HLA-E may be considered as an immune-evasive adaptation. Therefore, the non-classical major histocompatibility complex class Ib antigens HLA-G and HLA-E may be potential targets in the elimination of CRC-CSCs. However, more detailed studies are required to support our findings.

False positive results in diagnostic immunohistochemistry are related to horseradish peroxidase conjugates in commercially available assays

False positive results in diagnostic immunopathology can lead to unnecessary treatments. The purpose of this study was to do a side-by-side comparison of ten different antibodies commonly used in the clinical laboratory altering only the horseradish peroxidase (HRP) conjugate. The automated Leica BOND-MAX platform was used to study serial sections from 203 tissues including controls compared in a blinded fashion using the HRP conjugates from Leica (Refine HRP), Ventana Medical Systems (Ultraview HRP), and Enzo Life Sciences (Polyview HRP). False positive results, defined as signal from cases known to not contain the target, were noted in 23/171 cases (13%) with the Leica HRP, 62/171 cases (36%) with the Ventana HRP, and no cases with the Enzo HRP. Each data set was performed simultaneously allocating one tray for each of the three different HRP conjugates. HER2/neu analysis from triple negative breast cancers were scored as positive by immunohistochemistry in 6/25 cases (24%) using either the Refine or Ultraview HRP and in 0/25 cases with the Enzo conjugate. It is concluded that false positive results in a wide spectrum of diagnostic immunopathology tests can occur in from 13 to 36% of cases with two commonly used commercial assays based on the HRP conjugate.

Novel strategies for targeting leukemia stem cells: sounding the death knell for blood cancer
Abstract
Background
Cancer stem cells (CSCs), also known as tumor-initiating cells (TICs), are characterized by high self-renewal and multi-lineage differentiation capacities. CSCs are thought to play indispensable roles in the initiation, progression and metastasis of many types of cancer. Leukemias are thought to be initiated and maintained by a specific sub-type of CSC, the leukemia stem cell (LSC). An important feature of LSCs is their resistance to standard therapy, which may lead to relapse. Increasing efforts are aimed at developing novel therapeutic strategies that selectively target LSCs, while sparing their normal counterparts and, thus, minimizing adverse treatment-associated side-effects. These LSC targeting therapies aim to eradicate LSCs through affecting mechanisms that control their survival, self-renewal, differentiation, proliferation and cell cycle progression. Some LSC targeting therapies have already been proven successful in pre-clinical studies and they are now being tested in clinical studies, mainly in combination with conventional treatment regimens.
Conclusions
A growing body of evidence indicates that the selective targeting of LSCs represents a promising approach to improve disease outcome. Beyond doubt, the CSC hypothesis has added a new dimension to the area of anticancer research, thereby paving the way for shaping a new trend in cancer therapy.
