Beatboxers and Guitarists Engage Sensorimotor Regions Selectively When Listening to the Instruments They can Play

Abstract
Studies of classical musicians have demonstrated that expertise modulates neural responses during auditory perception. However, it remains unclear whether such expertise-dependent plasticity is modulated by the instrument that a musician plays. To examine whether the recruitment of sensorimotor regions during music perception is modulated by instrument-specific experience, we studied nonclassical musicians—beatboxers, who predominantly use their vocal apparatus to produce sound, and guitarists, who use their hands. We contrast fMRI activity in 20 beatboxers, 20 guitarists, and 20 nonmusicians as they listen to novel beatboxing and guitar pieces. All musicians show enhanced activity in sensorimotor regions (IFG, IPC, and SMA), but only when listening to the musical instrument they can play. Using independent component analysis, we find expertise-selective enhancement in sensorimotor networks, which are distinct from changes in attentional networks. These findings suggest that long-term sensorimotor experience facilitates access to the posterodorsal "how" pathway during auditory processing.

Primary aldosteronism (PA) is a severe and often underdiagnosed form of secondary hypertension. Determining the aldosterone to renin ratio (ARR) in hypertensive patients has been shown to be a valuable screening test for identification of patients suffering from PA. Since the introduction of a more widespread ARR screening the number of PA patients significantly increased worldwide. Interpretation of ARR might be challenging: Several factors from posture to interfering drugs affect the ARR and need to be taken into account when collecting samples. In addition, the wide variety of available assay methods and lack of well-established cut-offs present a challenge to the clinician. This review discusses the usefulness and possible difficulties of ARR screening.

Brain magnetic resonance imaging using a customized vacuum shape-keeping immobilizer without sedation in preterm infants

Abstract
Objective

To validate that a customized vacuum shape-keeping immobilizer (VaSKI) can be used in preterm infants without sedation.

Methods

Preterm infants who underwent brain MRI from February 2008 to March 2017 at Osaka Medical College Hospital were retrospectively assigned to the sedation group (February 2008 to September 2013, n = 64) or VaSKI group (October 2013 to November 2016, n = 64). The examination success rates and diagnosable case classification were determined by pediatricians and radiologists. We compared the time from preparation to examination completion between the groups. Furthermore, we measured the time from immobilization to sleep in the preterm infants in the VaSKI group.

Results

The examination success rate was 90.6% in both of the groups. The median (interquartile range) times of preparation were 24.0 (15.3) and 18.0 (13.0) min in the sedation and VaSKI groups, respectively. In the VaSKI group, the time from immobilization to sleep was within 3 min in 87.1% of the preterm infants.

Conclusions

The brain MRI examination success rates and motion artifact suppression effects were equivalent between the groups, whereas the time from preparation to examination completion was much shorter in the VaSKI group. Therefore, the VaSKI was validated as an immobilizer for MRI in preterm infants without using sedation.

Development of a robust diffusion-MR elastography (dMRE) technique to mitigate intravoxel phase dispersion

Abstract

Diffusion-magnetic resonance elastography (dMRE) is an emerging practical technique that can acquire diffusion magnetic resonance imaging and MRE simultaneously. However, a signal loss attributable to intravoxel phase dispersion (IVPD) interferes with the calculation of the apparent diffusion coefficient (ADC). This study presents an approach to dMRE that reduces the influence of IVPD by introducing a new pulse sequence. The existing and proposed techniques were performed using a phantom comprising five rods with different elasticities at 60 Hz vibration to investigate the accuracy of previous and proposed dMRE techniques. The measures of ADC and stiffness, obtained by using both dMRE techniques, were compared with conventional spin-echo (SE) diffusion and SE-MRE. Then, we evaluated those differences by using the mean of absolute differences (MAD) in each rod within the phantom. The results of the MAD of the stiffness from both dMRE techniques showed almost no difference. In contrast, the value of the ADC MAD (MAD ≒ 0.16 × 10−3 mm2/s), obtained in the soft region within the phantom with the previous dMRE technique, was large. This value was about 2.7 times that of the value produced by the proposed dMRE technique. This difference must reflect the degree of influence of IVPD in both techniques. These results demonstrate that our dMRE technique is a robust method for addressing the signal loss attributable to IVPD.

Minimal number of gradient directions for robust measurement of spherical mean diffusion weighted signal

Abstract
Purpose

Determination of the minimum number of gradient directions (Nmin) for robust measurement of spherical mean diffusion weighted signal (S¯).

Methods

Computer simulations were employed to characterize the relative standard deviation (RSD) of the measured spherical mean signal as a function of the number of gradient directions (N). The effects of diffusion weighting b-value and signal-to-noise ratio (SNR) were investigated. Multi-shell high angular resolution Human Connectome Project diffusion data were analyzed to support the simulation results.

Results

RSD decreases with increasing N, and the minimum number of N needed for RSD ≤ 5% is referred to as Nmin. At high SNRs, Nmin increases with increasing b-value to achieve sufficient sampling. Simulations showed that Nmin is linearly dependent on the b-value. At low SNRs, Nmin increases with increasing b-value to reduce the noise. RSD can be estimated as σS¯N, where σ = 1/SNR is the noise level. The experimental results were in good agreement with the simulation results. The spherical mean signal can be measured accurately with a subset of gradient directions.

Conclusion

As Nmin is affected by b-value and SNR, we recommend using 10 × b / b1 (b1 = 1 ms/μm2) uniformly distributed gradient directions for typical human diffusion studies with SNR ~ 20 for robust spherical mean signal measurement.

A feasible study on using multiplexed sensitivity-encoding to reduce geometric distortion in diffusion-weighted echo planar imaging

Abstract
Purpose

Single-shot (SS) echo planar imaging (EPI) is commonly used for diffusion-weighted imaging (DWI) in radiation treatment planning. The geometric distortion of single-shot diffusion-weighted EPI (SS-DWEPI) limits its utility in precision radiotherapy. We aimed to investigate the use of multiplexed sensitivity-encoding DWEPI (MUSE-DWEPI) to reduce the geometric distortions to allow its use in brain radiotherapy.

Methods

In a phantom study, phantom diameters measured using MUSE-DWEPI and SS-DWEPI were compared and the percentages of geometric distortion (%GD) were calculated. The shifting vectors of control points were also plotted and calculated. In a patient study, ten patients (six with post-surgery glioma, four with brain metastases) requiring MRI were enrolled, and the image distortion levels in SS-DWEPI and MUSE-DWEPI were compared using T2 Periodically Rotated Overlapping Parallel Lines with Enhanced Reconstruction (T2 PROPELLER) images as the reference. The tumor targets and four brain regions were delineated based on T2 PROPELLER, SS-DWEPI, and MUSE-DWEPI, using the Dice similarity coefficient (DSC) and the Hausdorff distance (HD) to quantify the level of geometric distortion.

Results

In the phantom study, the most prominent image distortion was along the phase encoding direction in terms of %GD fluctuated from 7%–8% for SS-DWEPI (b = 0 s/mm2 and 1000 s/mm2) and fluctuated from 2%–3% for MUSE-DWEPI (b = 0 s/mm2 and 1000 s/mm2) with different positions and b values. The mean relative displacement of all control points was 4.45 ± 3.44 mm for SS-DWEPI b0 and 2.17 ± 1.9 mm for MUSE-DWEPI b0 close to the isocenter. Increasing the distance away from the isocenter in the z direction, the distortion increased more in SS-DWEPI. For all brain regions and targets involved, higher DSC values and lower HDs were obtained using MUSE-DWEPI than with SS-DWEPI (p < .01). The mean improvement in HD when switching from SS-DWEPI to MUSE-DWEPI was 3.65 ± 1.31 mm.

Conclusions

MUSE-DWEPI is an improvement upon SS-DWEPI in geometric distortion reduction, which might be a promising application strategy for DWI in radiotherapy.

Regular and Irregular Inflection in Down Syndrome – New Evidence from German

Abstract

The study aims to explore whether regular inflectional morphology is affected in children/adolescents with Down syndrome (DS). German past participle forms were elicited for ten regular and ten irregular inflected verbs as well as for five novel verbs. Data were collected from a group of 21 monolingual German children and adolescents with DS (chronological age M = 11;03 years) and a group of 21 typically developing monolingual German children (chronological age M = 4;03 years) matched in chronological age to the nonverbal mental age of the DS group (mental age DS group M = 4;05 years). Data analysis indicated that eight children/adolescents with DS displayed a deficit in acquiring the regular participle marker -t and in applying it as default inflection in German participle formation. In contrast, a group of 13 individuals with DS performed similar to the typically developing control children. They had successfully acquired the regular participle affix -t and readily applied it as default inflection to produce participles for irregular verbs and novel verbs. The data indicate that the acquisition of regular inflectional morphology is not outside the scope of individuals with DS and succeeded in many affected individuals. However, a substantial number of individuals with DS displayed a selective deficit with regular default inflection. The occurrence of a selective deficit with regular default inflection in individuals with DS supports dualistic views to inflection, according to which two different cognitive components are involved in regular and irregular inflection that can be selectively affected in language disorders.

Feasibility of radiotherapy in nonagenarian patients: a retrospective study

Abstract

Purpose

Specific information about radiation therapy in nonagenarians is limited. In order to shed more light on the feasibility of radiotherapy in this challenging subgroup, a retrospective study was performed.

Methods

The data of 93 consecutive patients receiving irradiation treatment at the Department of Radiation Oncology, Ordensklinikum Linz Barmherzige Schwestern between June 2005 and December 2016 were analyzed. Patient- and treatment-related factors were extracted from the patient records. Overall survival (OS) was defined as time from irradiation to death or last follow-up. The survival rates were analyzed using the Kaplan-Meier method and log-rank test.

Results

The study population of 93 patients was between 90 and 99 years old (median 91 years). It included 59 women (63%) and 34 men (37%). Of these, 38 (41%) received definitive radiotherapy, 14 (15%) received neoadjuvant or adjuvant radiotherapy, whereas a palliative regimen was prescribed in 44% of the cases (n = 41). In all, 79 patients (85%) were able to complete their prescribed course of radiotherapy. While 16 (17%) patients reported grade 2 toxicities or higher, 4 had ≥grade 3 side effects (4%). The median survival was significantly higher in patients treated with adjuvant, neoadjuvant or definitive radiotherapy (13.8 months) compared to patients treated with palliative radiotherapy (3.6 months; p < 0.001).

Conclusion

Even in patients managed without preradiotherapy comprehensive geriatric assessment, carefully planned fractionated radiotherapy was feasible and resulted in acceptable rates of acute toxicities.

Exposure to Environmental Tobacco Smoke and Prevalence of Atopic Dermatitis among Adolescents in Kuwait

Background: This study aimed to (i) assess the prevalence of exposure to environmental tobacco smoke (ETS) at homes, (ii) assess the prevalence of atopic dermatitis and (iii) evaluate the association between ETS exposure and atopic dermatitis status among adolescents. Methods: During October 2015, a cross-sectional study design was implemented using a self-administered, modified version of the ISAAC (International Study of Asthma and Allergies in Childhood) questionnaire among students enrolled in nine high schools of one of the six governorates of Kuwait. Prevalence of ETS exposure at home (1 or more smokers at home vs. none) and atopic dermatitis were computed. A multivariable log-binomial regression model was used to evaluate the adjusted association between ETS exposure and atopic dermatitis. Results: Of 800 invited students, 746 (93.3%) participated, of whom 74.8% were Kuwaiti and 50.1% were female with a mean (SD) age of 16.8 (0.68) years. Prevalence of ETS exposure at home was 54%. Prevalence of atopic dermatitis was 14.9%. A final multivariable log-binomial regression model showed that compared with the adolescents free from atopic dermatitis, adolescents with atopic dermatitis were significantly more likely to have had ETS exposure at home (adjusted prevalence ratio = 1.49; 95% CI: 1.03–2.14; p = 0.034) or tended to be female (adjusted prevalence ratio = 1.42; 95% CI: 0.99–2.03; p = 0.054). Conclusions: A high prevalence of ETS exposure at home among adolescents was recorded. ETS exposure and female gender were significantly associated with atopic dermatitis status. Intervention fostering voluntary adoption of a smoke-free home rule may help protect the adolescents against ETS exposure and alleviate the menace of associated morbidity.
