Drug-induced expression of EpCAM contributes to therapy resistance in esophageal adenocarcinoma

Abstract

Background

With a less than 5% overall survival rate, esophageal adenocarcinoma (EAC) is one of the leading causes of death in the United States. Epithelial cell adhesion molecule (EpCAM) is a cancer stem cell (CSC) marker that is expressed in various epithelial carcinomas, including EAC. Accumulating evidence indicates that CSC subpopulations can initiate cancer development and, in addition, drive metastasis, recurrence and drug resistance. It has also been reported that EpCAM up-regulation in EAC may lead to an aggressive behavior and, thus, an adverse clinical outcome. Here, we aimed to determine whether treatment with standard chemotherapeutic agents may induce EpCAM expression and, concomitantly, increases in malignant potential and drug resistance in EAC.

Methods

EpCAM expression was assessed in 20 primary human EAC/adjacent normal tissues, as well as in a human EAC-derived cell line (OE-19), in a pre-malignant Barrett's Esophagus cell line (Bar-T) and in a benign esophageal cell line (HET 1-A), using immunohistochemistry, Western blotting and qRT-PCR, respectively. Drug-induced resistance was investigated in OE-19-derived spheres treated with (a combination of) adriamycin, cisplatin and 5-fluorouracil (ACF) using survival, adhesion and flow cytometric assays, respectively, and compared to drug resistance induced by standard chemotherapeutic agents (CTA). Finally, ACF treatment-surviving cells were evaluated for their tumor forming capacities both in vitro and in vivo using spheroid formation and xenograft assays, respectively.

Results

High EpCAM expression was observed in esophageal cancer tissues and esophageal cancer-derived cell lines, but not in adjacent benign esophageal epithelia and benign esophageal cell lines (HET 1-A and Bar-T). The OE-19 cell spheres were drug resistant and EpCAM expression was significantly induced in the OE-19 cell spheres compared to the non-sphere OE-19 cells. When OE-19 cell spheres were challenged with ACF, the EpCAM mRNA and protein levels were further up-regulated up to 48 h, whereas a decreased EpCAM expression was observed at 72 h. EpCAM down-regulation by RNA interference increased the ACF efficacy to kill OE-19 cells. Increased EpCAM expression coincided with the CSC marker CD90 and was associated with an aggressive growth pattern of OE-19 cell spheres in vivo.

Conclusions

From our data we conclude that an ACF-induced increase in EpCAM expression reflects the selection of a CSC subpopulation that underlies tumor development and drug resistance in EAC.

Complications, long-term outcome and quality of life following Surgisis® and muscle-covered implants in immediate breast reconstruction: a case-control study with a 6-year follow-up

Abstract

Background

Matrices are now commonly used in breast reconstruction, but the scientific evidence is still scares. The main aim was to compare complications and the need for corrections in immediate breast reconstruction with the porcine-derived Surgisis® with the traditional muscle-covered technique. The secondary aim was to compare long-term quality of life and satisfaction.

Methods

All consecutive patients who had their breast reconstructed with a Surgisis® or muscle-covered tissue expander/implant were included. Patients were followed clinically and with BREAST-Q.

Results

During the study period, 116 reconstructions (71 patients) were operated in the Surgisis® group and 132 reconstructions (90 patients) in the control group. The median follow-up time was 74 months (min 43–max 162). The total early complication rate was 37% in the Surgisis® group and 27% in the control group. There were no differences in implant loss (p = 0.68) or total number of complications (p = 0.24) between the two groups. Risk factors for complications were mainly patient characteristics and the use of a tissue expander. There was a slightly higher capsular contracture frequency in the Surgisis® patients (4.2% vs. 2.5%). The need for corrections and patient satisfaction and quality of life were similar in the two groups.

Conclusions

The use of Surgisis® in implant-based reconstruction seems to result in an acceptable total early complication rate. The rate might be higher in tissue expander-based reconstruction. Risk factors are mainly patient characteristics. The capsular contracture rate and need for corrections, as well as patient satisfaction and quality of life, are similar in the Surgisis® patients and muscle-covered controls.

Water quality assessment of the most important dam (Latyan dam) in Tehran, Iran

Abstract

A pattern of water quality affected by the input regime and site specification is one of the most important issues in water supply resources and demand management. The present study aims to assess the water quality of Latyan dam, the important reservoir that supplies drinking water of Tehran, the capital city of Iran. Monthly water sampling was performed at four depths of the dam. The physical and chemical properties of water including temperature, pH, turbidity, total dissolved solids (TDS), electrical conductivity (EC), hardness, as well as Ca2+, Mg2+, Na+, K+, Cl−, SO42−, silica, ammonium, nitrate, nitrite, and total phosphorus (TP) concentration, were monitored from May 2014 to January 2017. The results of statistical analysis indicated that the quality of water in the dam was affected by the depth in view of five variables of EC, pH, turbidity, SO42−, and TP (p ˂ 0.05). Moreover, it was determined that ten out of 17 variables including EC, turbidity, TDS, Ca2+, K, Cl−, SO42−, silica, nitrate, and TP were statistically significant (p ˂ 0.05) based on seasonal variation analyses. Comparing the mean values, it could be concluded that EC, TDS, Ca2+, K+, Cl−, and SO42− were slightly higher during the winter probably owing to the diluting effects of seasonal precipitation. Nutrient-like distribution, nitrate, and TP, as well as silica, were much higher during the spring representing a high-diatoms activity and eutrophication. Turbidity was higher in the fall compared to the other seasons, which also demonstrated rainfall effects. Evaluation of the aggressiveness index (AI) suggests that water is moderately corrosive, and noncorrosive in the winter compared to other seasons. Moreover, according to the findings of this research and their comparison with the national and international water quality guidelines/standards, it could be said that the water quality of Latyan dam during the study period was acceptable and thus it was usable as a drinking water supply. However, due to the changes occurred in nitrate and phosphorus and extensive development of eutrophication problem, accurate and continuous evaluation of water quality in this reservoir is deemed necessary.

Intraspecific variation in sensitivity of winter wheat ( Triticum aestivum L . ) to ambient ozone in northern China as assessed by ethylenediurea (EDU)

Abstract

Wheat is a major staple food and its sensitivity to the gas pollutant ozone (O3) depends on the cultivar. However, few chamber-less studies assessed current ambient O3 effects on a large number of wheat cultivars. In this study, we used ethylenediurea (EDU), an O3 protectant whose protection mechanisms are still unclear, to test photosynthetic pigments, gas exchange, antioxidants, and yield of 15 cultivars exposed to 17.4 ppm h AOT40 (accumulated O3 over an hourly concentration threshold of 40 ppb) over the growing season at Beijing suburb, China. EDU significantly increased light-saturated photosynthesis rate (Asat), photosynthetic pigments (i.e., chlorophyll and carotenoid), and total antioxidant capacity, while reduced malondialdehyde and reduced ascorbate contents. In comparison with EDU-treated plants (control), plants treated with water (no protection from ambient O3) significantly decreased yield, weight of 1000 grains, and harvest index by 20.3%, 15.1%, and 14.2%, respectively, across all cultivars. There was a significant interaction between EDU and cultivars in all tested variables with exception of Asat, chlorophyll, and carotenoid. The cultivar-specific sensitivity to O3 was ranked from highly sensitive (> 25% change) to less sensitive (< 10% change) by comparing the difference of the average grain yield of plants applied with and without EDU. Neither stomatal conductance nor antioxidant capacity contributed to the different response of the cultivars to EDU, suggesting that another mechanism contributes to the large variation in response to O3 among cultivars. Generally, the results indicate that present O3 concentration is threatening wheat production in Northern China, highlighting the urgent need for policy-making actions to protect this critical staple food.

Optimization process of organic matter removal from wastewater by using Eichhornia crassipes

Abstract

This study aimed to determine the optimal conditions for organic matter removal from wastewater by Eichhornia crassipes (E.C). As a matter of fact, a complete factorial design was used to determine the effect of residence time (X1), plant density (X2) and initial chemical oxygen demand (COD) concentration (X3) on the phytoremediation process. The process's performance was measured on COD (Y1), NH4+ (Y2) and PO43− (Y3), with the results indicating a reduction of 8.59–81.71% of COD (Y1); 22.53–95.81% of NH4+ (Y2) and 0.54–99.35% of PO43− (Y3). Then, the first-order models obtained for COD, NH4+ and PO43− removal were validated using different statistical approaches such as statistical and experimental validation. Moreover, multi-response optimization was carried out through different scenarios. On the whole, the results obtained indicated that two serial ponds are required for an optimum organic matter removal by Eichhornia crassipes. Indeed, for the first pond, a residence time of 15 days is needed with a plant density of 60 ft/m2 and an initial concentration of about 944 mg/L. The second was the same residence time as the first with similar plant density of 60 ft/m2 and an initial load 192 mg/L (> 200 mg/L). Optimal organic matter removal from wastewater using Eichhornia crassipes requires two ponds arranged in chain.

Radiation-Induced Optic Neuropathy: Clinical and Imaging Profile of Twelve Patients

Background: Radiation-induced optic neuropathy (RON) is a form of delayed radionecrosis of the anterior visual pathways, which develops within months to years after external cranial irradiation and causes severe and irreversible vision loss. Small series reports have adequately documented its clinical features, but imaging characteristics have been less completely described. Methods: We accrued cases from the University of Michigan Neuro-Ophthalmology Clinic files and from cases coded as "radiation optic neuropathy" at the University of Michigan Medical Center between 1994 and 2017. All patients had undergone 3D-conformal linear accelerator (photon) external beam radiation. We collected clinical details of vision loss, including the temporal relationship to radiation. A single neuroradiologist (E.A.L.) evaluated all available magnetic resonance imaging (MRI) studies, noting the presence of enhancement, expansion, or volume loss of the optic nerves or chiasm, corresponding T2 signal abnormalities, and the absence of demyelination or confounding compressive lesions. Results: Twelve patients (15 eyes) met inclusion criteria. Vision loss was usually monocular at outset, but both optic nerves were eventually involved in 3 (25%) patients. Although usually sudden in onset, vision loss often declined slowly over many months, frequently to finger counting, or worse without recovery. An afferent pupillary defect was always present at the time of presentation. Most affected optic discs were pale at the time of first visual symptoms, indicating that subclinical optic nerve damage had been present for several weeks. The latency from completion of radiation to onset of vision loss ranged from 7 to 48 months (average: 18 months). In 2 patients, radiation was delivered to the whole brain, rather than being limited to the anterior visual pathway. MRI typically displayed a discrete region of enhancement of the affected prechiasmatic optic nerve, often with expansion and high T2 signal in the enhancing segment. In 3 affected eyes, enhancement was apparent on imaging completed 3–6 weeks before the onset of vision loss. In one patient, segmental prechiasmatic enhancement became evident only on repeat MRI completed 7 months after vision loss. The duration of enhancement among 9 eyes with follow-up MRIs was at least 2 months, but in one case, enhancement was still present on a study performed 17 months after treatment. Conclusions: This study further delineates the profile of RON. Visual loss is often acute, profound, and monocular but may decline slowly after acute onset and later affect both optic nerves. High-resolution MRI of the optic nerves usually will display enhancement of a discrete segment of the intracranial prechiasmatic optic nerve, often with accompanying expansion and T2 hyperintensity. In some cases, these imaging features may precede vision loss. They may be subtle or appear after vision loss. Enhancement lingers for a wide interval, ranging in this study from 2 to at least 17 months. Recognition of these imaging characteristics assists in confirmation of the diagnosis of RON. Emergence delirium in children is related to epileptiform discharges during anaesthesia induction: An observational study

BACKGROUND Epileptiform discharges frequently occur in children during induction of anaesthesia. However, studies analysing the impact of epileptiform discharges on postoperative emergence delirium in children are still scarce. The aim of this study is to correlate the incidence of epileptiform activity during anaesthesia induction with the occurrence of emergence delirium during stay in the recovery room. OBJECTIVES Prospective, observational cohort study in children 0.5 to 8 years old undergoing planned surgery. Bifrontal electroencephalogram electrodes were placed before induction of anaesthesia. Visual electroencephalogram analysis was performed from start of anaesthetic agent administration until intubation with regard to epileptiform patterns: rhythmic polyspikes; periodic epileptiform discharges; delta with spikes; and suppression with spikes. Emergence delirium was assessed during stay in the recovery room using the Pediatric Assessment of Emergence Delirium Score. DESIGN Prospective, observational cohort study. SETTING University hospital – Germany/Berlin. Children were included between September 2015 and February 2017. PATIENTS A total of 62 Children, aged 0.5 to 8 years old undergoing planned surgery were included. MAIN OUTCOME MEASURES Primary outcome was emergence delirium. Secondary outcomes, peri-operative Electroencephalography (EEG) data analysis. The presented study analysed an association between emergence delirium and the occurrence of epileptiform discharges during anaesthesia induction. RESULTS A total of 43.5% of the children developed emergence delirium and 56.5% did not. Epileptiform discharges were observed more often in children developing emergence delirium (63%) compared with children not developing emergence delirium (43%). But only the occurrence of interictal spike events – such as rhythmic polyspikes; periodic epileptiform discharges and delta with spikes – were significantly related to emergence delirium (emergence delirium-group 48% vs. nonemergence delirium-group 14%, OR = 5.6 [95% CI: 1.7 to 18.7]; P = 0.004). CONCLUSION Emergence delirium in children is significantly related to interictal spike events occurring during induction of anaesthesia. CLINICAL TRIAL NCT02481999. Low-dose ketamine infusion reduces postoperative hydromorphone requirements in opioid-tolerant patients following spinal fusion: A randomised controlled trial

BACKGROUND The current opioid epidemic highlights the urgent need for effective adjuvant therapies to complement postoperative opioid analgesia. Intra-operative ketamine infusion has been shown to reduce postoperative opioid consumption and improve pain control in opioid-tolerant patients after spinal fusion surgery. Its efficacy for opioid-naïve patients, however, remains controversial. OBJECTIVE We hypothesised that low-dose ketamine infusion after major spinal surgery reduces opioid requirements in opioid-tolerant patients, but not in opioid-naïve patients. DESIGN Randomised placebo-controlled prospective study. SETTING Single-centre, tertiary care hospital, November 2012 until November 2014. PATIENTS A total of 129 patients were classified as either opioid-tolerant (daily use of opioid medications during 2 weeks preceding the surgery) or opioid-naïve group, then randomised to receive either ketamine or placebo; there were thus four groups of patients. All patients received intravenous hydromorphone patient-controlled analgesia postoperatively. INTERVENTION Patients in the ketamine groups received a ketamine infusion (bolus 0.2 mg kg−1 over 30 min followed by 0.12 mg kg−1 h−1 for 24 h). Patients in the placebo groups received 0.9% saline. MAIN OUTCOME MEASURES The primary outcome was opioid consumption during the first 24 h postoperatively. The secondary outcome was numerical pain scores during the first 24 h and central nervous system side effects. RESULTS Postoperative hydromorphone consumption was significantly reduced in the opioid-tolerant ketamine group, compared with the opioid-tolerant placebo group [0.007 (95% CI 0.006 to 0.008) versus 0.011 (95% CI 0.010 to 0.011) mg kg−1 h−1, Bonferroni corrected P

“A Review of Validated QOL PROMs in Pediatric Plastic Surgery.”

Background: There has been an exponential increase in the number of patient reported outcome measures, PROMs, in plastic surgery. This study aims to review the reliability, validity, and practicality of the most frequently used PROMs in pediatric plastic surgery research. Methods: A review of the literature from January 2010 - June 2015 was conducted to identify PROMs in pediatric plastic surgery. PROMs used in 5 articles with 2 validation studies were included for analysis and classified as generic, disease specific, and mental health. The type of validation used and reliability scores were compared across each class of PROMs. The practicality of each PROM was determined by the frequency of use and the number of items and cost. Results: Of the 173 unique PROMs identified, 14 were included for analysis and classified as: generic (7), disease specific (4), and mental health (3). The majority of all PROMs used construct validity. Disease specific PROMs had the highest distribution of domains related to physical functioning - the same domain also found to have the highest reliability scores. A PROM's frequency of use was not associated with its number of items or cost. Conclusions: This review found that generic PROMs were used most often, construct validity was used most frequently, physical functioning domains had the highest reliability, and the number of items or cost of a PROM was not related to its frequency of use. Taken together, this information may inform the future development or selection of PROMs in pediatric plastic surgery. Financial Disclosure Statement: The authors have nothing to disclose.
