Publication date: June 2018

Author(s): Stephan Meesters, Pauly Ossenblok, Albert Colon, Louis Wagner, Olaf Schijns, Paul Boon, Luc Florack, Andrea Fuster
ObjectiveThe interictal epileptic discharges (IEDs) occurring in stereotactic EEG (SEEG) recordings are in general abundant compared to ictal discharges, but difficult to interpret due to complex underlying network interactions. A framework is developed to model these network interactions.MethodsTo identify the synchronized neuronal activity underlying the IEDs, the variation in correlation over time of the SEEG signals is related to the occurrence of IEDs using the general linear model. The interdependency is assessed of the brain areas that reflect highly synchronized neural activity by applying independent component analysis, followed by cluster analysis of the spatial distributions of the independent components. The spatiotemporal interactions of the spike clusters reveal the leading or lagging of brain areas.ResultsThe analysis framework was evaluated for five successfully operated patients, showing that the spike cluster that was related to the MRI-visible brain lesions coincided with the seizure onset zone. The additional value of the framework was demonstrated for two more patients, who were MRI-negative and for whom surgery was not successful.ConclusionsA network approach is promising in case of complex epilepsies.SignificanceAnalysis of IEDs is considered a valuable addition to routine review of SEEG recordings, with the potential to increase the success rate of epilepsy surgery.

Telomere length, thyroid dysfunction/autoimmunity and parity

There may exist an association between thyroid dysfunction/autoimmunity and parity. Autoimmune thyroiditis shows some degree of telomere shortening. Parity was recently found to be associated with telomere shortening. We hypothesize that among the factors affecting the tentative association between parity and thyroid dysfunction/autoimmunity shortened telomeres' may also be implicated. This could also be another facet of the processes leading to autoimmune thyroiditis.

Vitamin D deficiency may stimulate fibroblasts in Dupuytreńs disease via mitochondrial increased reactive oxygen species through upregulating transforming growth factor-β1

Dupuytren's disease, a benign fibroproliferative disorder of the palmar fascia, represents an ideal model to study tissue fibrosis. Transforming growth factor-β1 (TGF-β1) and its downstream Smad signaling system is well established as a keyplayer during fibrogenesis. Vitamin D has been extensively studied as an anti-fibrotic agent in malignant chronic diseases. A number of studies have shown that myofibroblasts are main target cells of 1,25(OH)2D3 inhibitory action. The myofibroblast in the palmar aponeurosis of patients in different stages of Dupuytren's disease was found by electron microscopy to contain a large number of mitochondria. Mitochondria play a critical role in cell metabolism being the major source of reactive oxygen species (ROS) in cells. TGF-β1 has been shown to increase mitochondrial ROS production in different cell types, which mediate fibrosis related gene expression and myofibroblast differentiation. TGF-β1 increases mitochondrial ROS production in patients with Dupuytreńs contracture potentially in consequence of Vitamin D deficiency, leading to myofibroblast differentiation. Thus, targeting this basic pathomechanism seems suitable to establish new treatment strategies.

Neuromuscular taping reduces blood pressure in systemic arterial hypertension

S03069877.gif

Publication date: Available online 17 April 2018
Source:Medical Hypotheses
Author(s): M. Shah, P.O.O. Julu, J.A. Monro, J. Coutinho, C. Ijeh, B.K. Puri
Systemic arterial hypertension, a well-known cause of morbidity, is associated with dysfunction of the autonomic nervous system. Neuromuscular taping (also known as kinesio taping, medical taping and Vendje neuromuscular) allows movement and muscle activity to treat pain, muscle disorders and lymphoedema, in which its mode of action may involve muscular stimulation leading to increased local blood circulation or stimulating dermatological, muscular and fascial structures with a form of passive massage. We hypothesised that neuromuscular taping may reduce blood pressure in systemic arterial hypertension. This hypothesis was tested by carrying out the first pilot study of its kind to determine whether the non-invasive technique of neuromuscular taping can reduce blood pressure in patients suffering from systemic arterial hypertension. Neuromuscular taping was symmetrically applied to the back, between C1 and T2, of seven hypertensive patients for 5-7 days. Cardiovascular autonomic parameters were assessed at baseline and at the end of the study. Taping was associated with falls in mean arterial blood pressure (p=0.001), mean systolic blood pressure (p<0.01), mean diastolic pressure (p<0.01) and cardiac vagal tone at rest (p=0.063). The beneficial effects on blood pressure appeared to last for at least five days post-neuromuscular taping. There is an indication, given the reduction in cardiac vagal tone at rest, that the mechanism of action of this intervention involves modulation of the brainstem parasympathetic system during cardiovascular control. Further studies are indicated to replicate the present findings, further investigate the effects of taping on autonomic functioning, and establish the optimum time-period and taping positioning.

The electronics in fluorescent bulbs and light emitting diodes (LED), rather than ultraviolet radiation, cause increased malignant melanoma incidence in indoor office workers and tanning bed users

Publication date: Available online 17 April 2018
Source:Medical Hypotheses
Author(s): Samuel Milham, Dave Stetzer
The epidemiology of cutaneous malignant melanoma (CMM) has a number of facets that don't fit with sunlight and ultraviolet light as the primary etiologic agents. Indoor workers have higher incidence and mortality rates of CMM than outdoor workers; CMM occurs in body locations never exposed to sunlight; CMM incidence is increasing in spite of use of UV blocking agents and small changes in solar radiation.Installation of two new fluorescent lights in the milking parlor holding area of a Minnesota dairy farm in 2015 caused an immediate drop in milk production. This lead to measurement of body amperage in humans exposed to modern non-incandescent lighting. People exposed to old and new fluorescent lights, light emitting diodes (LED) and compact fluorescent lights (CFL) had body amperage levels above those considered carcinogenic. We hypothesize that modern electric lighting is a significant health hazard, a carcinogen, and is causing increasing CMM incidence in indoor office workers and tanning bed users. These lights generate dirty electricity (high frequency voltage transients), radio frequency (RF) radiation, and increase body amperage, all of which have been shown to be carcinogenic. This could explain the failure of ultraviolet blockers to stem the malignant melanoma pandemic. Tanning beds and non-incandescent lighting could be made safe by incorporating a grounded Faraday cage which allows passage of ultraviolet and visible light frequencies and blocks other frequencies. Modern electric lighting should be fabricated to be electrically clean.

Upregulated epithelial junction expression represents a novel parameter of the epithelial radiation response to fractionated irradiation in oral mucosa

Abstract

Purpose

During head and neck cancer treatment, the radiation response of the oral mucosa represents a frequent early side effect. Besides radiation-induced inhibition of proliferation, various other cellular responses occur. The radiation response of adherens and tight junction proteins was so far mostly investigated with large single-dose irradiation protocols, in vivo and in vitro. Therefore, the current study was initiated to investigate the impact of daily fractionated irradiation on the expression of adherens and tight junction proteins in vivo.

Materials and methods

Fractionation with 5 × 3 Gy/week (days 0–4, 7–11) was given to the snouts of mice. Groups of 5 animals per day were euthanized every second day between day 0 (unirradiated controls) and day 14, and their tongues subjected to histological processing. Adherens junction marker (β-catenin and E‑cadherin) and tight junction marker (claudin-1 and occludin) expression was analysed in the oral mucosa of unirradiated controls and during two weeks of fractionated irradiation.

Results

Adherens as well as tight junction marker proteins were rapidly and consistently upregulated in both the germinal as well as the functional layer of the oral mucosa. This represents a previously unknown parameter of the epithelial radiation response to clinically relevant fractionation protocols.

Conclusion

Fractionated irradiation significantly enhanced the expression of all proteins investigated. This study revealed a new parameter of the epithelial radiation response to fractionated irradiation.

Variation in accumulation, transport, and distribution of phthalic acid esters (PAEs) in soil columns grown with low- and high-PAE accumulating rice cultivars

Abstract

The extensive use of plasticizers containing di-n-butyl phthalate (DBP) and di-(2-ethylhexyl) phthalate (DEHP) results in high residual concentrations in agricultural soils and poses potential risks to human health through the food chain. Here, two rice cultivars with low (Fengyousimiao) and high (Peizataifeng) phthalic acid ester (PAE) accumulation were grown in leaching columns packed with DBP- and DEHP-contaminated soils to investigate their transport, fraction distribution, and accumulation in soil-rice-water system. Significant differences in soil vertical distribution of DBP and DEHP were observed among the two cultivars, sterilization and non-sterilization treatments. Both DBP and DEHP could leach to the bottom layer even though their concentrations in both soil and pore water decreased along with soil depth. DBP and DEHP concentrations in pore water were significantly correlated with those in corresponding soil layer at ripening stage. The available fractions including desorbing and non-desorbing fractions were predominant in the total concentrations of DBP and DEHP of soils. DBP and DEHP storages in coarse soil fractions (i.e., coarse and fine sands) with higher bioavailability displayed higher bioconcentration factors compared to finer soil fractions (i.e., silt and clay), and bioconcentration factors of Peizataifeng were higher than those of Fengyousimiao. The variations in vertical migration and accumulation of DBP and DEHP by the two cultivars implied different adverse effects on the security of groundwater and food.
