The free anterolateral thigh flap (ALT) is a versatile flap commonly used in reconstructive surgery. Its large skin territory, reliability, and versatility have made it one of the best choices for soft-tissue defect reconstruction. Its advantage over other flaps is its minimal donor site morbidity. Despite this, its adoption remains cautious because of several variations of its vascular anatomy. Rare but important donor site complication may occur and more scientific consistent documentation of donor-site outcomes is lacking.
