Novel strategy in giant cutaneous squamous cell carcinoma treatment: the case experience in a combination of photodynamic therapy and surgery

Case A 90-year-old Chinese man presented to our hospital with a one-year history of a large neoplasm on his right cheek (figure). The patient did not have subjective symptoms but emitted an obvious stench. However, the patient has significant embarrassment and difficult on social interaction due to abnormal appearance and foul smell from lesion. A clinical examination showed that the tumor size was 7cm * 6.5cm * 1.5cm. An X-ray examination showed that the deep tumor was not associated with any bone destruction. No abnormalities were found in other systems. Due to the advanced patient age, large tumor area, and deep infiltration, radical surgery could not initially be performed. After in situ resection, five rounds of topical 5-aminolevulinic acid photodynamic therapy (PDT) were continuously performed. The pathology examination showed the typical histology of squamous cell carcinoma. The surgical wound was healed five months after resection, and the residual tumor had a diameter of 0.8cm. The treatment was temporarily terminated due to a bone fracture. Seven months after resection, the diameter of the tumor focus increased to 2.5cm. At this time, we chose to perform a radical resection on the patient. After the surgery, a pathological examination of the resection margin and base showed no residual tumor. The patient was followed up for six months after the radical surgical resection and showed no tumor recurrence.
