To the Editor The article by Patel et al emphasizes the limitations of active surveillance in an intermediate-risk cohort of men with prostate cancer. It is particularly disappointing that there were no predictive features that allowed a prediction of favorable risk on final pathologic analysis in the low-volume Gleason 3 + 4 (grade group [GG] 2) cohort.
