The first dorsal metacarpal flap or Foucher flap is an island flap that was described as an alternative in the coverage of the distal stump or any other tissue loss in the thumb.1 The donor site is the skin from the dorsal aspect of the proximal phalanx of the index finger and second metacarpal head, which is usually grafted.
