The impact of IUGR on pancreatic islet development and {beta}-cell function

Placental insufficiency is a primary cause of intrauterine growth restriction (IUGR). IUGR increases the risk of developing type 2 diabetes mellitus (T2DM) throughout life, which indicates that insults from placental insufficiency impair β-cell development during the perinatal period because β-cells have a central role in the regulation of glucose tolerance. The severely IUGR fetal pancreas is characterized by smaller islets, less β-cells, and lower insulin secretion. Because of the important associations among impaired islet growth, β-cell dysfunction, impaired fetal growth, and the propensity for T2DM, significant progress has been made in understanding the pathophysiology of IUGR and programing events in the fetal endocrine pancreas. Animal models of IUGR replicate many of the observations in severe cases of human IUGR and allow us to refine our understanding of the pathophysiology of developmental and functional defects in islet from IUGR fetuses. Almost all models demonstrate a phenotype of progressive loss of β-cell mass and impaired β-cell function. This review will first provide evidence of impaired human islet development and β-cell function associated with IUGR and the impact on glucose homeostasis including the development of glucose intolerance and diabetes in adulthood. We then discuss evidence for the mechanisms regulating β-cell mass and insulin secretion in the IUGR fetus, including the role of hypoxia, catecholamines, nutrients, growth factors, and pancreatic vascularity. We focus on recent evidence from experimental interventions in established models of IUGR to understand better the pathophysiological mechanisms linking placental insufficiency with impaired islet development and β-cell function.

Conditional deletion of ELL2 induces murine prostate intraepithelial neoplasia

Elongation factor, RNA polymerase II, 2 (ELL2) is an RNA Pol II elongation factor with functional properties similar to ELL that can interact with the prostate tumor suppressor EAF2. In the prostate, ELL2 is an androgen response gene that is upregulated in benign prostatic hyperplasia (BPH). We recently showed that ELL2 loss could enhance prostate cancer cell proliferation and migration, and that ELL2 gene expression was downregulated in high Gleason score prostate cancer specimens. Here, prostate-specific deletion of ELL2 in a mouse model revealed a potential role for ELL2 as a prostate tumor suppressor in vivo. Ell2-knockout mice exhibited prostatic defects including increased epithelial proliferation, vascularity and PIN lesions similar to the previously determined prostate phenotype in Eaf2-knockout mice. Microarray analysis of prostates from Ell2-knockout and wild-type mice on a C57BL/6J background at age 3 months and qPCR validation at 17 months of age revealed a number of differentially expressed genes associated with proliferation, cellular motility and epithelial and neural differentiation. OncoPrint analysis identified combined downregulation or deletion in prostate adenocarcinoma cases from the Cancer Genome Atlas (TCGA) data portal. These results suggest that ELL2 and its pathway genes likely play an important role in the development and progression of prostate cancer.

Characterization of the human thyroid epigenome

The thyroid gland, necessary for normal human growth and development, functions as an essential regulator of metabolism by the production and secretion of appropriate levels of thyroid hormone. However, assessment of abnormal thyroid function may be challenging suggesting a more fundamental understanding of normal function is needed. One way to characterize normal gland function is to study the epigenome and resulting transcriptome within its constituent cells. This study generates the first published reference epigenomes for human thyroid from four individuals using ChIP-seq and RNA-seq. We profiled six histone modifications (H3K4me1, H3K4me3, H3K27ac, H3K36me3, H3K9me3, H3K27me3), identified chromatin states using a hidden Markov model, produced a novel quantitative metric for model selection and established epigenomic maps of 19 chromatin states. We found that epigenetic features characterizing promoters and transcription elongation tend to be more consistent than regions characterizing enhancers or Polycomb-repressed regions and that epigenetically active genes consistent across all epigenomes tend to have higher expression than those not marked as epigenetically active in all epigenomes. We also identified a set of 18 genes epigenetically active and consistently expressed in the thyroid that are likely highly relevant to thyroid function. Altogether, these epigenomes represent a powerful resource to develop a deeper understanding of the underlying molecular biology of thyroid function and provide contextual information of thyroid and human epigenomic data for comparison and integration into future studies.

The thermogenic effect of nesfatin-1 requires recruitment of the melanocortin system

Nesfatin-1 is a bioactive polypeptide expressed both in the brain and peripheral tissues and involved in the control of energy balance by reducing food intake. Central administration of nesfatin-1 significantly increases energy expenditure, as demonstrated by a higher dry heat loss; yet, the mechanisms underlying the thermogenic effect of central nesfatin-1 remain unknown. Therefore, in this study, we sought to investigate whether the increase in energy expenditure induced by nesfatin-1 is mediated by the central melanocortin pathway, which was previously reported to mediate central nesfatin-1's effects on feeding and numerous other physiological functions. With the application of direct calorimetry, we found that intracerebroventricular nesfatin-1 (25 pmol) treatment increased dry heat loss and that this effect was fully blocked by simultaneous administration of an equimolar dose of the melanocortin 3/4 receptor antagonist, SHU9119. Interestingly, the nesfatin-1-induced increase in dry heat loss was positively correlated with body weight loss. In addition, as assessed with thermal imaging, intracerebroventricular nesfatin-1 (100 pmol) increased interscapular brown adipose tissue (iBAT) as well as tail temperature, suggesting increased heat production in the iBAT and heat dissipation over the tail surface. Finally, nesfatin-1 upregulated pro-opiomelanocortin and melanocortin 3 receptor mRNA expression in the hypothalamus, accompanied by a significant increase in iodothyronine deiodinase 2 and by a nonsignificant increase in uncoupling protein 1 and peroxisome proliferator-activated receptor gamma coactivator-1 alpha mRNA in the iBAT. Overall, we clearly demonstrate that nesfatin-1 requires the activation of the central melanocortin system to increase iBAT thermogenesis and, in turn, overall energy expenditure.

Tunable magnetocaloric properties of Gd based alloys via Zn and Cd additions

Magnetocaloric effect by employing direct measurements of the adiabatic temperature changes (ΔTad) with a low magnetic field change (ΔH) up to 1.2T are investigated, for the Gd based alloys with Zn and Cd substituted for Gd individually or synchronously. A comparative study of the adiabatic temperature change (ΔTad) and magnetic entropy change (ΔSM) near room temperature has been reported in three different alloys of Gd1−xZnx, Gd1−yCdy and Gd0.95(Zn1−zCdz)0.05. Effects of Zn and Cd content on magnetocaloric effect and the Curie temperature (Tc) are studied in different alloys. A direct correlation between phase structure and ΔTad, ΔSM, Tc was discussed in three different types alloys of Gd1−xZnx, Gd1−yCdy, and Gd0.95(Zn1−zCdz)0.05. Mechanism of Zn improving the ΔTad and Cd increasing the Tc is revealed. It is expected that magnetic refrigeration near room temperature has a potential for superseding the traditional gas refrigeration.

Optimization and in line potentiometric monitoring of enhanced photocatalytic degradation kinetics of gemifloxacin using TiO 2 nanoparticles/H 2 O 2

Abstract

Gemifloxacin (GEM) is a broad-spectrum quinolone antibiotic. The presence of GEM residuals in industrial and hospital wastewater has been associated with genotoxicity and antibiotic resistance. In this contribution, the photodegradation of GEM using titanium dioxide nanoparticles (TiO2NPs)/H2O2 as a catalyst was optimized to eliminate residual drug and its photodegradates with antibacterial activity. A half-factorial design was implemented, investigating the effects of pH, initial concentration, H2O2 concentration, TiO2NP loading, and irradiation time. Owing to the time-dependent, multi-transformation of GEM into a wide range of structurally related photodegradation products, the monitoring of GEM throughout the experiments was achieved using both HPLC and potentiometric ion-selective electrodes (ISE). The sensor enabled in-line tracking of residual GEM in the presence of its photodegradates in real time. Results indicated that the pH, irradiation time, and GEM initial concentration were the most significant factors. At the optimum set of experimental conditions, the reaction followed first-order reaction kinetics with a mean percentage degradation of ~ 95% in less than 30 min of irradiation time and almost complete loss of antibacterial activity against Escherichia coli. The promising results demonstrated the efficiency of UV/TiO2NP/H2O2 as a photocatalyst for the breakdown of the pharmacophore of fluoroquinolones from water samples. The high selectivity, minimal solvent consumption, and lack of harmful waste generation confirmed the superiority of in-line monitoring using ISE. Optimization and in-line monitoring protocol should be applicable also at the pharmaceutical industry scale to eliminate the risk of antibiotic resistance.

On the eastern side of Lake Turkana in northern Kenya are extensive Plio-Pleistocene deposits containing a rich diversity of fossil mammals, hominins and flora within the radiometrically dated tuffaceous, lacustrine and fluvial sequence. Reconstruction of this landscape and paleoenvironment are part of an ongoing multinational and multidisciplinary human evolution project in the eastern Turkana Basin. Today there is a huge lake in the Rift Valley but it has fluctuated since the early Pliocene. Silicified wood is fairly common in some areas of the Koobi Fora Formation. One such site is FwJj 14E, alongside one of the tributaries of the Ileret River. Hominin hand and arm bones have been excavated from here in the Okote Member and dated at 1.56–1.36 Ma. The fossils are associated with hominin and bovid footprints. Sixty of the over 100 wood specimens collected have been sectioned and studied. In general the woods have large vessels and an average vulnerability index of 40, which implies a mesic megathermal environment with no water stress. Taxonomically the woods belong to large African families: Caesalpiniaceae (Didelotia idae), Combretaceae (Anogeissus sp.), Putranjivaceae (Euphorbiaceae; Drypetes sp.), Lamiaceae (cf Premna sp.), Malvaceae (Heritiera sp.) and Sapindaceae (Sapindoxylon sp.). Most of these taxa do not occur in the area today because now it is much drier and the local vegetation is predominantly Acacia-Commiphora-Salvadora shrubland. The reconstruction of the paleovegetation supports the interpretation from the fauna, namely, a tall riverine forest with shady refuge trees, possibly some edible fruits, and wooded grassland and more open bushland in the vicinity.

Teledermatology: A Review and Update

Abstract

Telemedicine is slowly transforming the way in which healthcare is delivered and has the potential to improve access to subspecialty expertise, reduce healthcare costs, and improve the overall quality of care. While many subspecialty fields within medicine today have either experimented with or begun to implement telemedicine platforms to enable remote consultation and care, dermatology is particularly suited for this care system as skin disorders are uniquely visible to the human eye. Through teledermatology, diagnostic images of skin disorders with accompanying clinical histories can be remotely reviewed by teledermatologists by any number of modalities, such as photographic clinical images or live video teleconferencing. Diagnoses and treatment recommendations can then be rendered and implemented remotely. The evidence to date supports both its diagnostic and treatment accuracy and its cost effectiveness. Administrative, regulatory, privacy, and reimbursement policies surrounding this dynamic field continue to evolve. In this review, we examine the history, evidence, and administrative landscape surrounding teledermatology and discuss current practice guidelines and ongoing controversies.

Data set for extraction and transesterification of bio-oil from Stoechospermum marginatum, a brown marine algae

The article presents the experimental data on the extraction and transesterification of bio-oil derived from Stoechospermum marginatum, a brown macro marine algae. The samples were collected from Mandapam region, Gulf of Mannar, Tamil Nadu, India. The bio-oil was extracted using Soxhlet technique with a lipid extraction efficiency of 24.4%. Single stage transesterification was adopted due to lower free fatty acid content. The yield of biodiesel was optimized by varying the process parameters. The obtained data showed the optimum process parameters as reaction time 90min, reaction temperature 65°C, catalyst concentration 0.50g and 8:1M ratio. Furthermore, the data pertaining to the physio-chemical properties of the derived algal biodiesel were also presented.

The elemental abundances (with uncertainties) of the most Earth-like planet

To first order, the Earth as well as other rocky planets in the Solar System and rocky exoplanets orbiting other stars, are refractory pieces of the stellar nebula out of which they formed. To estimate the chemical composition of rocky exoplanets based on their stellar hosts' elemental abundances, we need a better understanding of the devolatilization that produced the Earth. To quantify the chemical relationships between the Earth, the Sun and other bodies in the Solar System, the elemental abundances of the bulk Earth are required. The key to comparing Earth's composition with those of other objects is to have a determination of the bulk composition with an appropriate estimate of uncertainties. Here we present concordance estimates (with uncertainties) of the elemental abundances of the bulk Earth, which can be used in such studies. First we compile, combine and renormalize a large set of heterogeneous literature values of the primitive mantle (PM) and of the core. We then integrate standard radial density profiles of the Earth and renormalize them to the current best estimate for the mass of the Earth. Using estimates of the uncertainties in i) the density profiles, ii) the core-mantle boundary and iii) the inner core boundary, we employ standard error propagation to obtain a core mass fraction of 32.5 ± 0.3 wt%. Our bulk Earth abundances are the weighted sum of our concordance core abundances and concordance PM abundances. Unlike previous efforts, the uncertainty on the core mass fraction is propagated to the uncertainties on the bulk Earth elemental abundances. Our concordance estimates for the abundances of Mg, Sn, Br, B, Cd and Be are significantly lower than previous estimates of the bulk Earth. Our concordance estimates for the abundances of Na, K, Cl, Zn, Sr, F, Ga, Rb, Nb, Gd, Ta, He, Ar, and Kr are significantly higher. The uncertainties on our elemental abundances usefully calibrate the unresolved discrepancies between standard Earth models under various geochemical and geophysical assumptions.

Purpuric Agave Dermatitis

A 53 year old man presented with a 4 day history of a rapidly progressive, painful purpuric eruption on the lower extremities. The symptoms started as red to purple papules, some in a linear array, on the anterior and medial lower extremities several hours after working outside his house clearing foliage. The posterior legs were completely spared. Over the ensuing days, the lesions continued to progress, coalescing into large confluent purpuric plaques with surrounding purpuric papules and admixed multi-loculated hemorrhagic bullae (Figure 1). There were no lesions elsewhere on his body. He denied fever, chills, malaise, arthralgia, abdominal pain, hematuria, or other systemic symptoms. He had no significant medical history and took no medications.

Pseudoxanthoma elasticum-like changes in non-uremic calciphylaxis: case series and brief review of a helpful diagnostic clue

Calciphylaxis is a rare syndrome of vascular calcification with thrombosis that occurs most often in patients with end stage renal disease, and it frequently portends a guarded prognosis. Rarely, non-uremic calciphylaxis may occur; in this context, a strongly supportive histology is crucial in establishing the diagnosis. Herein, we describe two cases of non-uremic calciphylaxis associated with pseudoxanthoma elasticum-like changes, identified in both initial non-diagnostic and subsequent diagnostic biopsy specimens. This unusual but helpful histologic finding may support the early diagnosis and treatment of a potentially life-threatening disease in the context of subtle histopathologic vascular changes or in the absence of classic clinical or laboratory findings.

Diffusion-weighted magnetic resonance imaging in oral and maxillofacial lesions: preliminary study on diagnostic ability of apparent diffusion coefficient maps

Abstract

Objectives

The aim of this study was to investigate the usefulness of diffusion-weighted magnetic resonance imaging (DWI) in oral and maxillofacial lesions, especially the utility of apparent diffusion coefficient (ADC) maps for differential diagnosis of these lesions.

Methods

Fifty-seven patients who underwent magnetic resonance imaging for diagnosis of oral and maxillofacial lesions were included in this prospective study. DWI was performed on a 1.5 T unit with b-factors of 0 and 800 s/mm2, and ADC maps were generated. ADC values were measured for all 57 oral and maxillofacial lesions (19 squamous cell carcinoma, 10 medication-related osteonecrosis of the jaw, 6 odontogenic abscess, 4 ranula, 4 osteoradionecrosis, 4 hemangioma, 3 pleomorphic adenoma, 3 odontogenic keratocyst, 2 nasopalatine duct cyst, 1 malignant melanoma, and 1 basal cell carcinoma).

Results

The mean ADC values for ranula (2.69 ± 0.59 × 10−3 mm2/s) and nasopalatine duct cyst (2.34 ± 0.12 × 10−3 mm2/s) were significantly higher than those for the other oral and maxillofacial lesions (p = 0.000). In contrast, the mean ADC value for odontogenic abscess (0.67 ± 0.36 × 10−3 mm2/s) was significantly lower than those for the other oral and maxillofacial lesions (p = 0.000).

Conclusions

The present study suggests the usefulness of DWI in oral and maxillofacial lesions, especially the utility of ADC maps for differential diagnosis of these lesions.

Intravenous drug abuse is a risk factor in the failure of two-stage treatment for infected total hip arthroplasty

Reinfection after two-stage revision hip arthroplasty (RHA) is still a complex issue. Only few studies revealed the factors affecting the success rate in the treatment of periprosthetic hip infection (PHI), especially risk factors. A retrospective study was conducted using records of 30 patients underwent two-stage RHA for infected total hip arthroplasty (THA). Treatment was defined as successful if a patient did not need any reoperation or invasive procedure such as image-guided drainage during the two years after reimplantation. Treatment was defined as failure if any surgery or invasive procedure or long-term antibiotic suppression was considered necessary to control infection. Four patients had infection recurrence defined as failed and three of them had intravenous drug abuse. Twenty-six patients had no infection recurrence at the end of follow-up and one of them had intravenous drug abuse but quitting after surgery. We suggest that once adequate cleaning up achieved, risk of reinfection may be little even in immunocompromised patients with RHA because of relative less old age than those with revisional total knee arthroplasty. Patients of the reinfection group were younger and non-obese with adequate nutritional status. We may consider intravenous drug abuse could take a great toll on health and lead to reinfection. Finally, we suggest performing the gold-standard two-stage reimplantation technique to manage cases with infection, educating drug abusers regarding the risk of surgical failure, and implementing a quitting program at least 1 year before the index surgery.

Prolonged continuous exposure to high fine particulate matter associated with cardiovascular and respiratory disease mortality in Beijing, China

Although many studies examined the effects of fine particulate matter (PM2.5) on the deaths of cardiovascular disease (CVD) and respiratory disease (RD), few research has paid attention to the effects of prolonged continuous exposure to high PM2.5 pollution. This study estimated the excess risks (ER) of CVD and RD mortalities associated with prolonged continuous exposure to high PM2.5 pollution for the whole population and specific subsociodemographic groups in Beijing, which is the capital city of China with over 20 million residents and having severe PM2.5 pollution problems. Our results suggested that when high PM2.5 pollution occurred continuously, at various thresholds and durations, the adverse effects on CVD and RD mortalities varied significantly. The CVD mortality risks in association with prolonged continuous high PM2.5 pollution exposure were more serious for single individuals (including unmarried, divorced, and widowed), illiterate and outdoor workers than for other specific subsociodemographic groups. When the daily PM2.5 concentration higher than 105 μg/m3 consecutively occurs, at the ninth day, the ERs of CVD death for single individuals, illiterate and outdoor workers groups reached to 45% (95% CI: 22, 71), 51% (95% CI: 28, 79) and 53% (95% CI: 29, 82) respectively. On the other hand, prolonged continuous high PM2.5 pollution level appeared to contribute a higher proportion of RD deaths among illiterate and outdoor workers, but less significant for the other specific subsociodemographic groups. When the duration with daily PM2.5 pollution higher than 115 μg/m3 reached to six days, the ERs for outdoor workers and illiterate attributed to prolonged continuous PM2.5 pollution exposure increased 36% (95% CI: 5, 76) and 49% (95% CI: 16, 91) respectively.

The pulsating nature of large-scale Saharan dust transport as a result of interplays between mid-latitude Rossby waves and the North African Dipole Intensity

It was previously shown that during August the export of Saharan dust to the Atlantic was strongly affected by the difference of the 700-hPa geopotential height anomaly between the subtropics and the tropics over North Africa, which was termed the North African Dipole Intensity (NAFDI). In this work a more comprehensive analysis of the NAFDI is performed, focusing on the entire summer dust season (June–September), and examining the interactions between the mid-latitude Rossby waves (MLRWs) and NAFDI. Widespread and notable aerosol optical depth (AOD) monthly anomalies are found for each NAFDI-phase over the dust corridors off the Sahara, indicating that NAFDI presents intra-seasonal variability and drives dust transport over both the Mediterranean basin and the North Atlantic. Those summer months with the same NAFDI-phase show similar AOD-anomaly patterns. Variations in NAFDI-phase also control the displacement of the Saharan Heat Low (SHL) westwards or eastwards through horizontal advection of temperature over Morocco-Western Sahara or eastern Algeria-Western Libya, respectively. The connection between the SHL and the NAFDI is quantified statistically by introducing two new daily indexes that account for their respective phases (NAFDI daily index -NAFDIDI-, and SHL longitudinal shift index -SHLLSI-) and explained physically using the energy equation of the atmospheric dynamics. The Pearson's correlation coefficient between the one–day-lag SHLLSI and the NAFDIDI for an extended summer season (1980–2013) is 0.78. A positive NAFDI is associated with the West-phase of the SHL, dust sources intensification on central Algeria, and positive AOD anomalies over this region and the Subtropical North Atlantic. A negative NAFDI is associated with the East-phase of the SHL, and positive AOD anomalies over central-eastern Sahara and the central-western Mediterranean Sea. The results point out that the phase changes of NAFDI at intra-seasonal time scale are conducted by those MLRWs that penetrate deeply into the low troposphere.

Systematic ozone LIDAR measurements were completed during a 4 year period (2011–2014) in Paris, France to study the seasonal variability of the vertical structure of ozone in the urban boundary layer. In addition, we use in-situ measurements from the surface air quality network that is located in Paris (AIRPARIF). Specifically, we use ozone and NO2 measurements made at two urban stations: Paris13 (60 m ASL) and the Eiffel Tower (310 m ASL) to validate and interpret the LIDAR profiles. Remote sensed tropospheric NO2 integrated columns from the SAOZ instrument located in Paris are also used to interpret ozone measurements. Comparison between ozone LIDAR measurements averaged from 250 m to 500 m and the Eiffel Tower in-situ measurements shows that the accuracy of the LIDAR (originally ±14 μg·m−3) is significantly improved (±7 μg·m−3) when a small telescope with a wide angular aperture is used. Results for the seasonal cycle of the ozone vertical gradient are found to be similar using two methods: (1) measured differences between AIRPARIF stations with measurements at 60 m ASL and 310 m ASL and (2) using LIDAR profiles from 300 m to the top of the Planetary Boundary Layer (PBL). Ozone concentrations measured by the LIDAR increase with altitude within the PBL, with a steeper gradient in winter (60 μg·m−3·km−1) and a less strong gradient in summer (20 μg·m−3·km−1). Results show that in winter, there is a sharp positive gradient of ozone at the surface, which is explained by ozone titration by NO combined with increased atmospheric stability in winter. In the afternoon during summer, photochemistry and vertical mixing are large enough to compensate for ozone titration near the surface, where NOx is emitted, and there is no gradient in ozone observed. In contrast, in the summer during the morning, ozone has a sharper positive vertical gradient similar to the winter values. Comparison of the vertically averaged ozone concentrations up to (0–3 km) and urban layer (0–310 m) ozone concentrations shows that the ratio between these two quantities is the largest in summer (86%) and the lowest in winter (49%). We conclude that satellite measurements that represent the 0–3 km integrated ozone column are not necessarily a good proxy for surface ozone and may lead to incorrect conclusions about the surface ozone seasonal variability. The ratio between the urban layer NO2 average concentration and the boundary layer NO2 average concentration obtained from SAOZ NO2 tropospheric columns is always less than 50%, meaning NO2 does not decrease linearly in the PBL, but with a sharper decrease close to the surface.

Concentration of fine particulate matter (PM2.5) is one of the most important environmental parameters to estimate health impacts attributable to air pollution. Despite the fact there are many studies regarding PM2.5 effects on human health, most of them were performed under conditions that do not simulate the natural particles interaction with the organism. In the present paper, we studied the effects of mammals' sub-chronic exposure to PM2.5 on the lower respiratory tract, addressing realistic exposure conditions to normal urban air. Thus, we exposed Wistar rats under controlled settings to the same normal urban air, with and without particles. Next, we analyzed chemical composition of PM2.5 and lungs samples, performed a histologic examination and run the comet assay to assess genotoxic effects. We found a strong agreement between lung tissues and PM2.5 elemental composition suggesting that metals found in lungs came from the particles inhaled. Histological analysis showed a mild to moderate infiltration, with a reduction of alveoli lumen and increment of alveolar macrophages and periodic acid-Schiff (PAS) (+) cells in treated animals. We also observed an increase in the number of nuclei with comets, mostly comets type 3, with a high DNA fragmentation as well. These results provide strong evidence that sub-chronic exposure to low particle levels, even below the 24 h WHO standard, can cause injuries in lungs tissues and DNA damage, as well.

Eosinophil-rich trichoblastic carcinoma with aggressive clinical course in a young man

We present the case of a 35-year-old man who developed a follicular differentiated cutaneous carcinoma with an eosinophil-rich infiltrate and an aggressive clinical behavior. After an in-depth histopathological investigation the diagnosis of trichoblastic carcinoma was made. Over the course of the disease the patient developed a cutaneous in-transit metastasis as well as an axillary lymph node metastasis 18 months after the excision of the primary tumor on his back. Based on a literature review we discuss the different concepts behind the term "trichoblastic carcinoma" and we summarize the clinical and histological details of previously reported cases. Furthermore, we focus on the phenomenon of tumor-associated eosinophilia.

Evaluation of renal osteodystrophy in the dental clinic by assessment of mandibular and phalangeal cortical indices

Abstract

Objectives

Secondary hyperparathyroidism (SHPT) is a disease that affects patients with chronic kidney disease, and is characterized by mineral disturbance and bone loss, known as renal osteodystrophy. The aim of this study was to assess the validity of using intraoral phosphor storage plates to take radiographs of the middle phalanges to evaluate bone loss resulting from SHPT during follow-up of these patients.

Methods

The sample consisted of 24 patients with chronic kidney disease, 12 with parathyroid hormone (PTH) levels ≥500 pg/ml, and 12 with PTH levels <500 pg/ml, who underwent hemodialysis weekly. For each patient, a panoramic radiograph and digital radiographs of the ring, index, and middle fingers of both hands were taken. The Mandibular Cortical Index (MCI) and the Trabecular Bone Pattern Index (TBP) were applied to the panoramic radiographs, while the Phalangeal Cortical Index (PCI) was applied to the digital radiographs of the phalanges. Three evaluators performed all analyses.

Results

Significant correlations were found between the PTH levels and the MCI (p = 0.023), the PCI (p = 0.039) and the TBP index (p = 0.032). These parameters were also significantly interrelated (MCI × PCI = 0.001; MCI × TBP = 0.004 and PCI × TBP = 0.009). The PCI was shown to have the highest correlation with PTH levels.

Conclusion

In patients with chronic renal disease, it is clinically relevant to use panoramic and digital radiographs using intraoral storage plates to assess a number of quantitative parameters that can be linked to PTH levels.

Relationship between the retinacula cutis and sagging facial skin

Abstract

Background

Sagging skin is one of the most concerning esthetic issues for elderly individuals. Although reduced skin elasticity has been reported as the cause of sagging skin, a loss of skin elasticity alone is insufficient to explain sagging facial skin. This study investigated the mechanisms underlying sagging skin, with a focus on the subcutaneous network of collagenous fibers known as the retinacula cutis (RC).

Methods

To evaluate the structure of the RC noninvasively, tomographic images of the face were obtained using magnetic resonance imaging (MRI). The RC was identified by comparing MRI results with histological specimens of human skin. A descriptive scale was used to evaluate the degree of sagging, and a device equipped with a 6-mm-diameter probe was used to measure the elasticity of deeper skin layers and evaluate the physical properties of the skin.

Results

The density of RC in subcutaneous tissue correlated negatively with sagging scores and positively with elasticity.

Conclusion

These results imply that a sparse RC structure contributes to a reduction in the elasticity of subcutaneous tissue, resulting in a greater degree of sagging facial skin. These findings are expected to contribute to the understanding of the mechanisms underlying sagging skin.

The double-pedicled dorsal-metacarpal-artery (dpDMCA) flap of the hand: a novel DMCA-derived flap

Abstract

The dorsal-metacarpal-artery (DMCA) flap in its standard or extended version is considered as the working horse to cover dorsal soft tissue finger defects with exposed extensor tendon or bone. We hereby present a clinical case of an 80-year-old male patient who is right-handed and sustained a soft tissue defect of the proximal dorsal aspect of his left 5th finger and the postoperative outcome employing a modified transposition flap. The double-pedicled DMCA flap (dpDMCA flap) of the hand poses in adequate clinical scenarios a comparably fast and safe solution to cover dorsal finger defects extending just distal to the PIP joint. To the best of our knowledge, this is the first report of a DMCA-based flap with a double pedicle to cover soft tissue defects at the dorsum of the hand.

Stereotactic Radiosurgery for Intractable Tremor-Dominant Parkinson Disease: A Retrospective Analysis

Objective: The purpose of this study was to retrospectively analyze the outcomes of stereotactic radiosurgery for patients suffering from medically refractory Parkinson disease (PD) tremor. Methods: We retrospectively studied the outcomes of 33 patients who were treated with gamma knife thalamotomy (GKT) over a 19-year period. Twelve patients were ≥80 years. A median dose of 140 Gy (range, 130-150 Gy) was delivered to the nucleus ventralis intermedius through a single 4-mm isocenter. We used the Fahn-Tolosa-Marin clinical tremor rating scale to score tremor, handwriting, drawing, and ability to drink fluids. The median time to the last follow-up was 23 months (range, 9-144 months). Results: After GKT, 31 patients (93.9%) experienced improvement in tremor. Twenty-three patients (70.0%) had complete or nearly complete tremor arrest. Nine patients (27.2%) noted tremor arrest and resolution of impairment in writing, drawing, and ability to drink fluids. One patient (3%) improved in bradykinesia, 3 patients (9%) improved in rigidity, and 3 patients (9%) decreased their dosage of dopa after GKT. Tremor relief was fully maintained in the last follow-up for 96.8% of responding patients. Two patients (6%) experienced temporary adverse radiation effects. Conclusions: GKT is a safe and effective treatment for medically refractory PD tremor, especially for the elderly or those not suitable for deep brain stimulation or thermal thalamotomy.

IgE auto-reactivity in bullous pemphigoid: eosinophils and mast cells as major targets of pathogenic immune reactants

Abstract

Background

Bullous pemphigoid (BP) is an autoimmune disease characterised by tense blisters that are usually preceded by urticarial eruptions. Affected patients exhibit IgG and/or IgE auto-antibodies against BP180 and/or BP230. Their relative importance in disease pathogenesis has not been fully elucidated.

Objectives

The aim of this study was to better characterise circulating and tissue-resident IgE in BP patients at the serological, structural and functional level.

Methods

Sera (n=19) and skin (n=33) from BP patients were analysed via ELISA and immunofluorescence, respectively.

Results

Results obtained show that BP patients exhibit elevated IgE levels in the serum and in the skin. Within the latter, it is very rarely and only sparsely found along the basement membrane zone (BMZ), but is prominently present on mast cells and eosinophils. At least a portion of these IgE antibodies are BP-specific, as evidenced by serum ELISA and by the co-localization of BP180 and FcεRI-bound IgE on mast cells and/or eosinophils. An important role of these immune reactants can be implied by our additional finding that cross-linking of IgE, derived from BP sera, on FcεRI-expressing rat basophils with BP180 results in robust degranulation of these cells.

Conclusion

We propose the existence of a disease pathway alternative to IgG and complement that may well be responsible for some of the clinical features of this autoimmune disease.

A 5-year randomized trial on the safety and efficacy of pimecrolimus in atopic dermatitis: a critical appraisal

Summary

Aim

The PETITE study (Sigurgeirsson et al.) aimed to compare safety and efficacy of pimecrolimus 1% cream (PIM) and low-to-medium-potency topical corticosteroids (TCS) in children with mild-to-moderate atopic dermatitis (AD).

Setting and design

Participants of this 5-year drug-company sponsored multicentre, open-label, parallel-group trial were recruited between April 2004 and October 2005. No details are reported regarding the study sites.

Study exposure

Infants aged ≥ 3 to < 12 months with mild-to-moderate AD were randomly assigned in a 1 : 1 ratio to receive either PIM or a low- or medium-potency TCS cream/ointment for 5 years. No information on specific TCS products used was provided. The topical treatment was applied twice daily 'until complete AD clearance or for as long as allowed by the label of the specific TCS', and was reinitiated at the occurrence of first signs and symptoms of AD flares. In the PIM group, exacerbations not controlled by PIM were treated with short-term TCSs.

Outcomes

Adverse events (AEs) and serious AEs (SAEs) were recorded 'during clinic visits'. In a proportion of the patients, various immunological assessments including antibody titres to common vaccine antigens, immunoglobulin levels, B and T lymphocyte cell counts, and T-cell proliferation tests were performed. The children's growth was assessed by measuring height and weight. AD severity was measured using the Investigator Global Assessment (IGA) score and the percentage of the total body surface area affected. No specific information was provided on the number and scheduling of study visits. Primary outcomes were the incidence of AEs 'of primary clinical interest' and those with a crude incidence of ≥ 5% in either treatment group. Secondary outcome was 'long-term efficacy' defined as IGA ≤ 1 at week 3 and year 5.

Results

Patients in the PIM group experienced significantly more AEs [bronchitis (P = 0·02), infected eczema (P ≤ 0·001), impetigo (P = 0·045), nasopharyngitis (P = 0·04)]. No significant differences were seen for the other AEs. The overall incidence of SAEs was slightly higher for PIM (20·5% vs. 17·3%; P = 0·046). The proportion of participants with IGA ≤ 1 at year 5 was 88·7% for PIM and 92·3% for TCS, a success rate difference of 3·6% (95% confidence interval 0·8–6·4) favouring TCS.

Conclusions

Sigurgeirsson et al. conclude that the long-term management of mild-to-moderate AD in children with both TCS and PIM is safe, and that PIM has similar efficacy to TCS. Further, they conclude that their data support the use of PIM as a first-line treatment of mild-to-moderate AD in children.

Diagnostic Accuracy of Lymphoscintigraphy for Lymphedema and Analysis of False-Negative Tests

imageBackground: Lymphedema is the chronic enlargement of tissue due to inadequate lymphatic function. Diagnosis is made by history and physical examination and confirmed with lymphoscintigraphy. The purpose of this study was to assess the accuracy of lymphoscintigraphy for the diagnosis of lymphedema and to determine characteristics of patients with false-negative tests. Methods: Individuals referred to our lymphedema program with "lymphedema" between 2009 and 2016 were analyzed. Subjects were assessed by history, physical examination, and lymphoscintigraphy. Patient age at presentation, duration of lymphedema, location of disease, gender, previous infections, and lymphedema type were analyzed. Results: The study included 227 patients (454 limbs); lymphedema was diagnosed clinically in 169 subjects and confirmed by lymphoscintigraphy in 162 (117 primary, 45 secondary; 96% sensitivity). Fifty-eight patients were thought to have a condition other than lymphedema, and all had negative lymphoscintigrams (100% specificity). A subgroup analysis of the 7 individuals with lymphedema clinically, but normal lymphoscintigrams, showed that all had primary lymphedema; duration of disease and infection history were not different between true-positive and false-negative lymphoscintigram results (P = 0.5). Two patients with a false-negative test underwent repeat lymphoscintigraphy, which then showed lymphatic dysfunction consistent with lymphedema. Conclusion: Lymphoscintigraphy is very sensitive and specific for lymphedema. All patients with false-negative studies had primary lymphedema. A patient with a high clinical suspicion of lymphedema and a normal lymphoscintigram should be treated conservatively for the disease and undergo repeat lymphoscintigraphy.

Maximum entropy models as a tool for building precise neural controls

Neural responses are highly structured, with population activity restricted to a small subset of the astronomical range of possible activity patterns. Characterizing these statistical regularities is important for understanding circuit computation, but challenging in practice. Here we review recent approaches based on the maximum entropy principle used for quantifying collective behavior in neural activity. We highlight recent models that capture population-level statistics of neural data, yielding insights into the organization of the neural code and its biological substrate. Furthermore, the MaxEnt framework provides a general recipe for constructing surrogate ensembles that preserve aspects of the data, but are otherwise maximally unstructured. This idea can be used to generate a hierarchy of controls against which rigorous statistical tests are possible.

The construct of coercive control has been central to many conceptualizations of intimate partner violence (IPV), yet there is widespread inconsistency in the literature regarding how this construct is defined and measured. This article provides a comprehensive literature review on coercive control in regards to conceptualizations, definitions, operationalization, and measurement; and attempts to provide a synthesis and recommendations for future research. A summary and critique of measures used to assess coercive control in IPV is provided. At least three facets of coercive control are identified: 1) intentionality or goal orientation in the abuser (versus motivation), 2) a negative perception of the controlling behavior by the victim, and 3) the ability of the abuser to obtain control through the deployment of a credible threat. Measurement challenges and opportunities posed by such a multifaceted definition are discussed.

The acute toxicity of bisphenol A and lignin-derived bisphenol in algae, daphnids, and Japanese medaka

Abstract

Risk assessing newly synthesized chemicals prior to their applications is extremely important, if we want to ensure substitution of risky chemicals with more benign ones. During the past two decades, many analogs of bisphenol A (BPA) have been manufactured, while their toxicity remains less studied. The aim of this study was to compare the acute toxicity of a synthesized lignin-derived BPA (LD-BP) with that of BPA in representative aquatic organisms including two algal species (Chlorella pyrenoidosa and Scenedesmus obliquus), a cladoceran species (Daphnia magna), and the Japanese medaka (Oryzias latipes). The results revealed that the two algal species showed different responses to the two chemicals. For C. pyrenoidosa, both BPA and LD-BP stimulated growth within 48 h of exposure, except for the 50 mg L−1 of LD-BP treatment. After 96 and 144 h of exposures, BPA stimulated the growth of C. pyrenoidosa at low-exposure concentrations but inhibited its growth at high concentrations, while LD-BP caused a concentration-dependent response in C. pyrenoidosa. S. obliquus exhibited a monotonic concentration-response curve for both BPA and LD-BP exposures. For both D. magna and O. latipes, concentration-responses were monotonic with 96 h–LC50 of BPA and LD-BP of 11.7 and 5.0 mg L−1 and 9.4 and 4.1 mg L−1, respectively. Our results demonstrate that LD-BP is more toxic than BPA in the representative aquatic organisms, and it can pose higher ecological risk to the aquatic ecosystem than BPA.

Acculturative stress as a moderator of the effect of drinking motives on alcohol use and problems among young adults

BackgroundResearch has demonstrated a strong relationship between drinking motives and alcohol use among young adult populations. Further, there is substantial evidence of the association between psychosocial stressors and greater alcohol consumption. In the present study, we examined whether acculturative stress would moderate the relationship between major drinking motives and alcohol use behaviors and alcohol-related problems, and whether this relationship differs by racial/ethnic group.MethodSix hundred diverse undergraduate students (mean age=21.50, SD=2.46; 82.8% female; 40.2% White/Caucasian) completed a series of measures, including demographic information, alcohol use/alcohol problems, and acculturative stress.ResultsFindings showed varying patterns in the moderating effect of acculturative stress on the relationship between drinking motives and alcohol consumption and alcohol-related problems, particularly for self-identified Hispanic, Black, and Asian students. Stronger drinking motives were associated with greater alcohol consumption and alcohol-related problems, and this relationship was stronger among Asian and Hispanic students who reported higher levels of acculturative stress. For Black students, greater acculturative stress was observed to weaken the relationship between coping, enhancement, and conformity motives and alcohol drinking frequency.ConclusionsResults are discussed in terms of incorporating acculturative stress in the development of tailored alcohol use interventions for vulnerable young adults, such as certain ethnic minority and immigrant groups. Clinical implications include screening for acculturative stress in primary care and counseling centers and early intervention programs to identify young adults who may be at-risk for or currently experiencing alcohol problems.

The aim of the current paper was to explore the experiences that might lead people to reduce their alcohol consumption and to compare these findings between respondents from 21 different countries.MethodsGlobal Drug Survey is an online cross sectional, opportunistic anonymous survey. This paper includes 72,209 respondents from 21 counties with over 250 respondents (60.8% male).ResultsAlmost a third (32.9%) of participants reported that they would like to drink less alcohol over the next 12months, and a third thought their GP would tell them to cut down if they were honest about their drinking. The primary experiences that were rated as most likely to lead to a change in behaviour were related to physical health, sexual assault and having to seek emergency medical treatment. Respondents from Germany were more likely to select embarrassment as a motivation to reduce drinking than those from other counties. Females were more likely to report indicate motivations related to sexual regret, sexual assault or seeking treatment. Older participants and those in the low risk audit category were more likely to report embarrassment or forgetfulness as potential motivation for change.ConclusionUnderstanding the different motivations that may lead individuals to change their drinking behaviours can be used to inform targeted brief interventions and targeted public health guidance.

Themes arising in clinical consultation for therapists implementing family-based treatment for adolescents with anorexia nervosa: a qualitative study

Abstract

Background

Our study aims to explore and describe themes arising in sessions of clinical consultation with therapists implementing Family-Based Treatment (FBT) for adolescents with Anorexia Nervosa (AN). There is currently no literature describing the content of clinical consultation for FBT. Thus, this knowledge will add to the evidence-base on what therapists need from consultants in ongoing clinical consultation.

Methods

Eight therapists at four sites participated in this study, which spanned a two-year period. Following a two-day training workshop, each therapist treated at least one adolescent patient presenting with a restrictive eating disorder with FBT, focusing on adherence to the treatment manual. Clinical consultation sessions occurred monthly and were led by an external FBT expert. Thirty-five (average per site = 9) audio recorded group clinical consultation sessions were transcribed verbatim and coded for themes. Twenty percent of the transcripts were double-coded to ensure consistency. Fundamental qualitative description guided the sampling and data collection.

Results

Thematic content analysis revealed ten common themes relating to the provision of clinical consultation to therapists implementing FBT in clinical practice: encouraging parental meal time supervision,discussing the role of mothers, how to align parents, ensuring parental buy-in, when to transition to Phase 2, weighing the patient and the patients' knowledge of their weight, the role of siblings in FBT sessions, how best to manage patient co-morbidities, the role of the father in FBT and how best to manage the family meal.

Conclusions

In conclusion, clinical consultation themes aligned with many of the central tenets of FBT, including how to help parents align their supportive approach during the refeeding process, and how to help parents assume control of eating disordered behaviours. This knowledge helps to guide consultants to anticipate common issues brought forward by therapists attempting to implement FBT.

A novel technique for correcting the over-corrected gynaecomastia using stacked acellular dermal matrix

Abstract

The 'saucer-shaped deformity' is the undesirable outcome following over-resection of gynaecomastia. Correction can be challenging, lipomodelling may be unfavourable or unpredictable, and autologous grafts often require additional unwanted scars. We present a case of unilateral gynaecomastia over-resection where disks of acellular dermal matrix (ADM) were interwoven in the subdermal, intrapectoral, and interpectoral planes to correct the areolar cavity. A routine review at 17 months showed maintained projection of the areola, and the patient was delighted with the natural contour.

Amyloid β (Aβ) is the major component of senile plaques (SP) in the brains of Alzheimer's disease (AD) patients, and serves as an inflammatory stimulus for microglia. Trans-caryophyllene (TC), a major component in the essential oils derived from various species of medicinal plants, has displayed its neuro-protective effects in previous studies. However, whether TC has a protective role in AD remains unknown. In this study, the effects of TC on Aβ1–42-induced neuro-inflammation were investigated. We found that TC reduced the release of LDH in BV-2 microglial cells treated with Aβ1–42. In addition, pretreatment of BV2 microglia with TC at concentrations of 10, 25, and 50μM prior to Aβ stimulation led to significant inhibition of nitric oxide (NO) and prostaglandin E2 (PGE2) production, expression of inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2), and secretion of pro-inflammatory cytokines. Notably, our results indicate that TC remarkably attenuated Aβ1–42-activated overexpression of toll-like receptor 4 (TLR4). We further demonstrated that TC markedly reversed Aβ1–42-induced phosphorylation and degradation of IκBα, nuclear translocation of p65, and NF-κB transcriptional activity. These findings suggest that TC may have therapeutic potential for the treatment of AD.

BackgroundVisceral pleural invasion (PL) is a prognostic factor in lung cancer. In the lung, lymph flows along the pleura, in addition to the flow toward the pulmonary hilum just as the pulmonary arteries and veins run toward it. Even with the same tumor diameter, a PL1 or higher level of pleural invasion is indicative of a more advanced disease stage. Final diagnosis based on the PL level is made by pathological examination of excised specimens. However, if an intraoperative diagnosis can be established, proper selection of the surgical procedure can be made, and unnecessary surgeries for disseminated lesions can be avoided. We investigated optical diagnostic techniques for identifying the presence or absence of visceral pleural invasion in lung cancer by capitalizing on the phenomenon of 5-amino-levulinic acid (5-ALA) being metabolized to a photosensitizing substance or protoporphyrin IX within malignant tumors, generating red luminescence in response to excitation light.MethodThis study included 38 patients with primary lung cancer who underwent surgery. They received 5-ALA (20mg/kg) orally 4h before surgery and then we assessed the presence or absence of pleural invasion using an autofluorescence observation system. At visceral pleural invasion sites, we were able to confirm tumor sites visualized in red with a clear border in contrast to the green autofluorescence generated in normal tissues.ResultRed luminescence could be confirmed in 100% of PL1-PL3 patients (14/14) and 41.6% of PL0 patients (10/24) with primary lung cancer. PL0 patients in whom visualization was possible were preoperatively diagnosed as having PL1 and many of them showed vascular channel invasion. The sensitivity, specificity, positive predictive value, and negative predictive value of this diagnostic technique were 100%, 58.0%, 63.1%, and 100%, respectively. Red fluorescence emission was observed significantly more often in pleural invasion cases.ConclusionAccurate intraoperative diagnosis for visceral pleural invasion in lung cancer may contribute to determining the indications for limited operations such as segmental resection. In addition, accurate local diagnosis has the possibility of being applicable to photodynamic therapy.

Primary Cutaneous NK/T-cell Lymphoma of Nasal Type: An Age-related Lymphoproliferative Disease?

Among extranodal NK/T-cell lymphoma of nasal type (NKTL), the extranasal variant (ENKTL) is known to have a worse prognosis with advanced clinical stage than the nasal variant of NKTL. However, detailed clinicopathological features of the localized extranasal disease have not been well documented in English literature. Here, we described the clinicopathological profiles of 14 patients with stage I ENKTL, including 7 in the skin, 5 in the gastrointestinal tract, and 2 in the central nervous system, highlighting the distinctiveness of the first. The 7 primary cutaneous (PCNKTL) cases were characterized by an older onset age (median, 76 vs. 53years, P=.012) and a more favorable clinical course (P=.041), compared to 17 patients with stages II-IV ENKTL that showed cutaneous involvement. The skin lesions in the PCNKTL group were distributed in the face or neck (n=4) and limbs (n=3), but not the trunk, which was most frequently affected (60%, P=.017) in the latter group. Furthermore, the stage I cutaneous disease showed a female predominance (M:F, 2:5 vs. 7:0, P=.021) and a significantly more favorable survival compared to the non-cutaneous stage I ENKTL (P=.037). These results suggested that PCNKTL constituted a distinct subgroup in the nasal type lymphoma spectrum.

Toxic pyrrolizidine alkaloids provide a warning sign to overuse of the ethnomedicine Arnebia benthamii

Ethnopharmacological relevanceFrom early times man has used medicinal plants for the treatment of various ailments and basic health care needs. The use of herbal medicines has increased day by day and with this, so do reports of adverse events, poisoning, and suspected toxicity. Similarly, the indigenous communities of Neelum Valley in Azad Kashmir commonly use Arnebia benthamii (Wall. ex G.Don) I.M.Johnst. for medicinal purposes to treat various human aliments. Besides their medicinal uses, it also contains hepatotoxic pyrrolizidine alkaloids (PAs).Aim of the studyThis explorative study underscores two major aspects about this herbal medicine. Firstly we aimed to document the traditional therapeutic uses of Arnebia benthamii in Neelum Valley, Azad Kashmir. Secondly, to determine the presence or absence of hepatotoxic pyrrolizidine alkaloids and if they are within the suggested limit for the use of herbs in excess.Materials and methodsInterviews, group discussions, and inquiries were carried out from July to September 2016 with local indigenous and elder people. In the laboratory, the plant was investigated for pyrrolizidine alkaloids by using high performance liquid chromatography (HPLC).ResultsA total of 30 respondents were interviewed. They explained the preferred preparation, parts used, and treatment indications. Treatment of fever along with kidney and liver problems are the three principle uses. Among the different parts of Arnebia benthamii, 43% respondents preferred aerial parts for the herbal formulation, followed by whole plants, and leaves. Decoction was the major mode of preparation and all herbal preparations were administrated orally. This study reports, for the first time according to our literature review, a study of Arnebia benthamii with regard to PA determination. By using column Zorbax SB-Aq and acetonitrile-water gradient as the mobile phase, HPLC results showed that the aerial parts of the plant were PA positive, and (1) Europine, Heliotrine (2), Lycopsamine (3), and Echimidine (4) were identified.ConclusionsThis study has revealed two new findings of significance to herbal medicine producers, practitioners, and consumers of Arnebia benthamii First, local knowledge regarding the medicinal uses of Arnebia benthamii were documented in five sites of Neelum Valley, Azad Kashmir. The use of this plant by a large part of the population in the study area shows the importance for their therapeutic benefits. Unfortunately, the second finding of this study shows that Arnebia benthamii contains hepatotoxic PAs. Hence, we advised to the government regulatory authorities and non-governmental organizations that use of this plant as herbal medicine should be excluded before more accurate quality control tests.

Thalidomide alleviates postoperative pain and spatial memory deficit in aged rats

Pain is a major risk factor of post-operative cognitive dysfunction (POCD) in aged population. We investigated the effects of thalidomide, an anti-inflammatory and analgesic drug, on POCD in aged rats, and also explored the underlying mechanisms. Laparotomy was performed under anesthesia in aged rats (24–25 months) to establish POCD models. Thalidomide (5–50mg/kg) was intraperitoneally administered immediately after laparotomy. Within 12h after the operation, pain symptoms were assessed by rat grimace scale (RGS). Within postoperative day (POD) 3–14, spatial memory was evaluated using performance errors in a radial arm maze. Protein levels of inflammatory cytokines and N-methyl-D-aspartate (NMDA) receptors were measured on POD 14. POCD rats treated with thalidomide showed decreased RGS and performance errors, compared with saline-treated POCD rats. Single administration of thalidomide significantly reduced production of cytokines (tumor necrosis factor (TNF)-α and interleukin (IL)-1β) in serum but not in the brain, and attenuated upregulation of NMDA receptor (NR) 2A/B subunits in the hippocampus at POD 14. MK-801 abolished thalidomide-induced attenuation of spatial memory deficits. Our results support that thalidomide could disrupt the development of post-operative memory deficit in aged rats through its long-term regulation of NMDA receptors (NRs) in the hippocampus. Therefore, thalidomide might provide a new means to prevent the development of POCD.

3-O-Glucosylation of quercetin enhances inhibitory effects on the adipocyte differentiation and lipogenesis

Glycosylation of natural flavonoids with various sugar moieties can affect their physicochemical and pharmacological properties. In this study, the plant flavonoids quercetin aglycon (Quer) and quercetin 3-O-glucoside (Q3G) were evaluated and compared for their potential anti-obesity effects. The Q3G dose-dependently reduced the TG contents and lipid accumulation in 3T3-L1 adipocyte cells, by 52% and 60% at 20μM, respectively, compared to differentiated control (100%), which were 1.6-fold and 2.4-fold higher reduction than Quer. The Q3G (20μM) also more significantly reduced the expression of adipogenic markers such as C/EBP-β, C/EBP-α, PPAR-γ, and aP2 than Quer, indicating that the Q3G suppresses both adipocyte differentiation and lipogenesis more effectively than Quer in vitro. Comparing to those in the high-fat diet (HFD) fed mice control group for 10 weeks, both the body and liver weights and the size of adipocytes in epididymal adipose tissues were significantly reduced in HFD mice fed with Q3G for another 6 weeks (30mg/kg body weight by oral administration), accompanied by the reductions of TG, total cholesterol, and HDL-cholesterol in serum. The Q3G also reduced the levels of the lipid metabolism-associated proteins, PPAR-γ, SREBP-1c, and FAS in the liver tissues. These results clearly demonstrated that Q3G exhibits a stronger anti-obesity effect than Quer and its anti-obesity effect is mediated via inhibition of adipocyte differentiation and lipogenesis, decreasing serum lipid levels by altering hepatic lipid metabolism, and reducing body weight gain. The results of this study suggest that the Q3G, but not Quer, can be a potent functional ingredient of beneficial health foods or a therapeutic agent to prevent or treat obesity.

ObjectiveDisruption in the neural activation of the prefrontal cortex (PFC) in modulating arousal was explored in children with heavy prenatal alcohol exposure (PAE), who have known neurobehavioral impairment.MethodsDuring a task that elicits frustration, functional near-infrared spectroscopy (fNIRS) was used to measure PFC activation, specifically levels of oxygenated (HBO) and deoxygenated (HBR) hemoglobin, in children with PAE (n=18) relative to typically developing Controls (n=12) and a Clinical Contrast group with other neurodevelopmental or behavioral problems (n=14).ResultsChildren with PAE had less activation during conditions with positive emotional arousal, as indicated by lower levels of HBO in the medial areas of the PFC and higher levels of HBR in all areas of the PFC sampled relative to both other groups. Children in the Control group demonstrated greater differentiation of PFC activity than did children with PAE. Children in the Clinical Contrast group demonstrated the greatest differences in PFC activity between valences of task conditions.ConclusionsSpecific patterns of PFC activation differentiated children with PAE from typically developing children and children with other clinical problems.SignificanceFNIRS assessments of PFC activity provide new insights regarding the mechanisms of commonly seen neurobehavioral dysfunction in children with PAE.

ObjectivePerformance monitoring, which allows efficient behavioral regulation using either internal (error processing) or external (feedback processing) cues, has not yet been explored in binge drinking despite its adaptive importance in everyday life, particularly in the regulation of alcohol consumption. Capitalizing on a theoretical model of risky behaviors, the present study aimed at determining the behavioral and electrophysiological correlates of the cognitive (inhibition) and motivational (reward sensitivity) systems during performance monitoring.MethodsEvent-related potentials were recorded from 20 binge drinkers and 20 non-binge drinkers during two experimental tasks, a speeded Go/No-Go Task [investigating internal error processing by Error-Related Negativity (ERN) and error positivity (Pe)] and a Balloon Analogue Risk Task [investigating external feedback processing by Feedback-Related Negativity (FRN) and P3].ResultsWhile no group differences were observed at the behavioral level, electrophysiological results showed that binge drinkers, despite having intact feedback-related components, presented modified error-monitoring components (i.e. larger ERN amplitude, delayed Pe latency).ConclusionsInternal performance monitoring is impaired in binge drinkers, showing an abnormal automatic processing of response errors (ERN) and a decreased processing of their motivational significance (Pe).SignificanceThese results suggest that the electrophysiological correlates of inhibitory control allow identifying the specific binge drinking consumption pattern.

FGF2-mediated attenuation of myofibroblast activation is modulated by distinct MAPK signaling pathways in human dermal fibroblasts

BackgroundPrevious human and animal studies have demonstrated the ability of exogenously administered basic fibroblast growth factor (FGF2) to act as an antifibrotic agent in the skin. Though the activity of FGF2 as an anti-scarring agent is well-established for fibrotic skin wounds, the mechanisms by which FGF2 exerts these actions are not entirely understood. Canonical FGF2 signaling proceeds in part via FGFR/MAPK pathways in human dermal fibroblasts, and FGF2 has been described to prevent or reverse the fibroblast-to-myofibroblast transition, which is driven by TGFβ signaling and understood to be an important step in the formation of a fibrotic scar in vivo. Thus, we set out to investigate the antagonistic effects of FGF2 on TGFβ signaling as well as the broader effects of MAPK inhibition on the TGFβ-mediated induction of myofibroblast gene expression.ObjectiveTo better understand the effects of FGF2 signaling pathways on myofibroblastic gene expression and cell phenotypes.MethodsHuman dermal fibroblasts were cultured in vitro in the presence of FGF2, TGFβ, and/or MAPK inhibitors, and the effects of these agents were investigated by molecular biology techniques including qRT-PCR, immunofluorescence, Western blot, and flow cytometry.ResultsFGF2 inhibited TGFβ-mediated fibroblast activation, resulting in more rapidly proliferating, spindle-shaped cells, compared to the more slowly proliferating, flatter TGFβ-treated cells. Treatment with FGF2 also attenuated TGFβ-mediated increase in expression of myofibroblast markers smooth muscle α-actin, calponin, transgelin, connective tissue growth factor, ED-A fibronectin, and collagen I. FGF2-mediated antagonism of the TGFβ-mediated fibroblast-to-myofibroblast transition was reversed by small molecule inhibition of ERK or JNK, and it was potentiated by inhibition of p38. MAPK inhibition was demonstrated to have qualitatively similar effects even in the absence of exogenous FGF2, and small molecule inhibition of p38 MAPK was sufficient to attenuate TGFβ-mediated fibroblast activation.ConclusionsInhibition of select MAPK signaling pathways can reverse or potentiate anti-fibrotic FGF2 effects on human dermal fibroblasts, as well as exert their effects independently of exogenous FGF2 supplementation.

Differential expression of CCR4 in primary cutaneous gamma/delta (γ⁄δ) T cell lymphomas and mycosis fungoides: significance for diagnosis and therapy

Primary cutaneous γ⁄δ T cell lymphomas (PCγδTCL) are rare and usually aggressive T cell lymphomas. Expression of markers potentially useful for therapy, such as CCR4 and other molecules implicated in Th-like immune response may help to better understand these tumors and has not been reported to date.Ten PCγδTCL (5 showing epidermotropism) and 10 MF skin biopsies from 19 patients were studied. Immunohistochemical expression of CCR4, CXCR3, GATA-3, and IL-17 was examined and scored using quantitative and semi quantitative methods <ABS-P>CCR4 was expressed in 10/10 (100%) of MF and in 1/9 (11%) PCγδTCL cases (p=0.001). No significant difference in CXCR3 and GATA-3 expression was found between MF and PCγδTCL, although PCγδTCL did not display a Th2-like phenotype, contrary to MF. IL-17 showed very low (<20%) staining in most of the cases, with detection of rare strongly positive cells in 3 cases of PCγδTCL.Lack of CCR4 expression in PCγδTCL, including epidermotropic variants, suggests that these patients are less favorable candidates for therapy with mogamulizumab. Differential expression of CCR4 and absence of Th2-like immune profile may support the contention that epidermotropic PCγδTCL and MF are indeed biologically distinct. Increased levels of mitochondrial DNA copy number in patients with vitiligo

Summary

Background

Oxidative stress is known to be involved in the pathogenesis of autoimmune diseases such as vitiligo. Evidence suggests that the human mitochondrial DNA copy number (mtDNAcn) is vulnerable to damage mediated by oxidative stress. The purpose of this study was to examine and compare peripheral blood mtDNAcn and oxidative DNA damage byproducts (8-hydroxy-2-deoxyguanosine; 8-OHdG) in patients with vitiligo and healthy controls (HCs).

Methods

The relative mtDNAcn and the oxidative damage (formation of 8-OHdG in mtDNA) of each sample were determined by real-time quantitative PCR. Blood samples were obtained from 56 patients with vitiligo and 46 HCs.

Results

The mean mtDNAcn and the degree of mtDNA damage were higher in patients with vitiligo than in HCs.

Conclusion

These data suggest that increase in mtDNAcn and oxidative DNA damage may be involved in the pathogenesis of vitiligo.

Effects of pH, initial Pb 2+ concentration, and polyculture on lead remediation by three duckweed species

Abstract

Various geographical duckweed isolates have been developed for phytoremediation of lead. The Pb2+ removal efficiency of Lemna aequinoctialis, Landoltia punctata, and Spirodela polyrhiza was investigated in monoculture and polyculture at different levels of pH and initial Pb2+ concentrations. L. aequinoctialis was not sensitive to the tested pH but significantly affected by initial Pb2+ concentration, whereas synergistic effect of pH and initial Pb2+ concentration on removal efficiency of L. punctata and S. polyrhiza was found. Although the majority of polycultures showed median removal efficiency as compared to respective monocultures, some of the polycultures achieved higher Pb2+ removal efficiencies and can promote population to remove Pb2+. Besides, the three duckweed strains could be potential candidates for Pb2+ remediation as compared to previous reports. Conclusively, this study provides useful references for future large-scale duckweed phytoremediation.

Effect of culturing temperatures on cadmium phytotoxicity alleviation by biochar

Abstract

Biochar produced from rice straw at 400 °C (RS400) was prepared to determine its alleviating effect on Cd phytotoxicity to wheat seedlings under different cultivation temperatures and pH. A hydroponic system (pH 4.3) and a loam soil slurry system were designed to respectively simulate acidic and neutral soil condition, and cultivation at increasing temperatures (20, 25, and 30 °C) were performed to evaluate the greenhouse effect. The root and shoot elongation and the Cd concentration in root and solution were measured; furthermore, batch experiments for Cd adsorption were undertaken. An increasing inhibition of the root by Cd addition was observed at increasing temperatures. The inhibition rate was 50.50 and 20.80% in hydroponic system and slurry system at 25 °C, respectively; however, the corresponding inhibition rates of root were significantly decreased to 25.5 and 3.5% with addition of RS400. This is mainly attributed to the reduction of Cd migration into the roots by RS400, which decreased Cd bioavailability. The mechanism behind the reduced Cd bioavailability is attributed to the Cd adsorption and the strong buffering capacity of acidity by RS400. Therefore, biochar could be a potential amendment for the remediation of Cd-contaminated soil even at increasing culturing temperatures.

Bacillus safensis with plant-derived smoke stimulates rice growth under saline conditions

Abstract

Salinity is a worldwide environmental problem of agricultural lands. Smoke and plant growth-promoting bacteria (PGPR) are individually used to improve plant growth, but the combined effects of these have not been studied yet under saline conditions. The combined effect of plant growth-promoting bacteria Bacillus safensis and plant-derived smoke Cymbopogon jwarancusa was studied under different salinity level as 50, 100, and 150 mM on rice (cv. Basmati-385). Smoke dilutions of C. jwarancusa (C-500 and C-1000) and bacterial culture of B. safensis were used to soak seeds for 10 h. It was observed that the salt concentration decreases the germination percentage, vegetative growth, ion contents (K+ and Ca2+), and photosynthetic pigments (Chl "a," Chl "b," and carotene) while an increase occurred in Na+, total soluble protein (TSP), proline, total soluble sugar, catalase (CAT), and peroxidase (POD) contents. The combined effect of B. safensis and smoke primed seeds increased the germination percentage, seedling growth, ion contents (K+, Ca2+), and photosynthetic pigments (Chl "a," Chl "b," carotene) and reduced the Na+ ion content, total soluble protein, proline content, total soluble sugar, CAT, and POD activity by lowering the drastic effect of salt stress. It was concluded that combined effect of smoke and PGPR is more effective than individual effect.

Comprehensive treatment for patients with chronic pain in a 12-step based substance use disorder program

Patients with chronic pain (CP) and substance use disorder (SUD) are complex, not yet adequately described, and in need of comprehensive treatments that address both diseases concurrently. Our objectives were to (a) describe a cohort of CP patients who failed traditional treatment (mainly opioids) – then developed opioid use disorder (OUD) and other SUDs and (b) evaluate a comprehensive inpatient treatment program for these patients. Patients were enrolled in an inpatient CP program. Treatment consisted of medical detoxification, group process/education, external and internal qigong, osteopathic manual medicine, and qigong-based mindfulness. Patients also received 20 h per week of 12-step recovery-based SUD treatment. Patients were evaluated at the beginning of treatment (first assessment, day 1–5 range) and at days 30 and 45. Assessments were: Beck Depression Inventory-II, Brief Pain Inventory, West Haven-Yale Multidimensional Pain Inventory (patient section), and McGill Pain Questionnaire. The Global Impression of Change Scale was administered at day 45. A mixed model analysis was used to evaluate treatment progress. Demographic data revealed an older cohort with OUD, other SUDs, and multiple pain diagnoses who failed traditional treatment. Questionnaire evaluations were consistent and similar across all of the above measures: patients' scores showed marked, statistically significant improvements in depression, pain, and quality of life ratings. The most substantial improvements occurred between the first and second assessments. The findings are sufficiently encouraging to warrant further evaluation of the protocol and to plan comparative studies.

Co-contaminant effects on ofloxacin adsorption onto activated carbon, graphite, and humic acid

Abstract

Given their voluminous application, significant amounts of fluoroquinolones are discharged into the environment through wastewater effluent. Adsorption has been shown to be a critical process controlling the environmental behaviors of fluoroquinolones. Competition between ofloxacin (OFL) and naphthalene (NAP)/bisphenol A (BPA) and their adsorption on activated carbon (AC), graphite (GP), and humic acid (HA) were investigated. The suppressed adsorption of OFL was observed on AC and GP, but not on HA, by NAP or BPA. Moreover, for AC, the competition by NAP was slightly stronger than that by BPA. However, for GP, the competition with BPA was higher than that with NAP. These observations indicate that competitive adsorption of OFL with respect to NAP/BPA depends on the degree of overlap of adsorption sites, as interpreted by the following: (i) AC can provide overlapping adsorption sites for OFL, BPA, and NAP, which include non-specific adsorption sites, such as hydrophobic sites, π-π interactions, and micropore filling; (ii) π-π interactions and hydrogen bonding might be responsible for the strong competitive adsorption between BPA and OFL on GP; and (iii) OFL adsorbs on HA through specific adsorption force—electrostatic attraction, with which NAP and BPA cannot compete.
