I must declare at the outset that I am a fan of the Furlow procedure, as are the authors. My initial misgivings that it was not an anatomical repair were rapidly over come with experience, not only because it has a high success rate and a very low complication rate but also that it can be performed with out the need of an operating microscope, using loupes, and so can be taught and used across the developing world. The finding in this paper that age at which an SMCP is repaired is a factor in outcome is proving a 'given fact' for most cleft surgeons.
